Reason unknown trucker climbed upon floats for dredge after they were loaded on a flat bed trailer. He either slipped, tripped or stepped off the float and fell approx 8 1/2 ft to ground striking his head and face on the ground. He was bleeding from nose & mouth also unconscious for approx 3 minutes then confused for 20 minutes.

Employee was getting in jon boat, was walking to the rear of the boat to operate the boat motor, employee tried to step on the metal seat to get to the back of boat, when he stepped on the metal seat he slipped, hitting the left side of his torso on the seat/bottom of the boat.

Removing pipe from pump - the pipe wrap fell hitting right thumb.

EE Got off the Trackhoe to help load the pipe into the Fusing Machine. His Foot got stuck in the mud causing him to lose his balance. Upon losing his balance, his left ankle rolled to the inside causing the injury

Employee aligning large pipe (12" to 14") diameter to install a tee in the pipe line - was using a chain and Komatsu-wa2oo, put self in twisted postion to assist in the lift.

He was changing oil in PC450 Komatsu Excavator. He was standing beside engine when he went to pull down hood to close, hand slipped off causing him to fall backwards into ringer oil container, causing pain to his lower back.

Employee is scheduled to see ear, nose & throat (ENT) doctor on 10/4/11 to determine if the hearing shift is work related if the hearing shift is work related ee exposure will be evaluated to determine the contributing conditions.

Over exertion by employee.

Employee was working to rewire VFD control box for electric booster station pump. Employee reached inside the VFD circuit box to check the wiring and must have hit a bare wire causing an ignition or spark to occur that caused him to have burns to his face/head/neck/arms.

Draining pipe from screen bowl to froth feed and got spider bit on right hand.

EE was assisting B & G service technician with replacement of impeller & housing on #5 underflow booster pump. EE was standing 4' from service truck when lightning struck the CB antenna on truck knocking EE to ground.

Heavy rain and deep mud, the employee was going up and down on the track hoe, when he slipped on the steps of the track hoe and caught himself with right hand and straining his right shoulder.

Ratchet binder slipped causing ee to mash thumb on right hand and possible dislocating.

Employee was replacing a brass bushing for the propeller shaft on the work boat using an 8 ton hydraulic jack. When he released the jack it slipped pinching his right hand index finger resulting in a cut requiring stitches.

EE was replacing a wear liner in #1 jig feed bin. As he was welding it in, the polished steel was reflecting back into the hood from the sides and behind him causing the arc burn to his eyes.

EE was removing the hydraulic rams from an old crusher and was using a hammer to drive out the pins holding it in place. He missed the pin, hitting is left wrist with the hammer.

Winter conditions/mud-snow, icy, worker had mud on his boots causing him to slip while going down the stairs.

Using a pry bar to clear rocks and wood from under a leaking 14" water line. Pushing back and forth on pry bar when it shifted sending employee forward into the catwalk sliding his little finger down the edge slicing his little finger open.

employee was tightening the bolts on the pipe line for the dredge and pulled his shoulder and bicep of his left arm.

Bag house #1 tube had coal fines builing up and falling down chunks into the Jig, EE was reaching in and pulling out the chunks of coal fines and strained his neck.

EE was assisting with maintenance on a pump, he was trying to clean around the pump to make it more accessible for other EE's working on pump. Pushed shovel to dig and when it stopped against surrounding material, his weight shifted against shovel and lost balance. His body weight shifted, felt pull in his leg. Trouble with leg due to prior injury not related to work.

He was climbing down the ladder of the WA 500 with wash hose in hand and slipped hitting his left knee on the step below.

Cleaning up mud & spillage around plant feed belt with skidsteer - put foot on bucket to exit the machine and right foot slipped and he fell over bucket - right knee hyper extended

10dB STS on right ear from annual evaluation on 9/16/09 and a 25dB reportable loss since baseline evaluation on 12/10/07.

EE had finshed lunch and went up in the plant to clean out the #1 baghouse hopper with an air lance which is a air hose with a 1/2 inch pipe hooked to it with a ball valve to turn air on and off with the valve. EE was removing the lance from the hopper it hit him in the mouth knocking out a lower front tooth. The ball valve is what struck him knocking the tooth out.

EE overloaded shovel, it was heavier than he expected due to the difference in material (coal,fines,rocks,clays). EE was cleaning material from the conveyor where it was clogged up.

Weather was changing - becoming very windy w/gusts to 40 mph - trying to pull doors (large metal) closed on 2nd floor of plant using a large wooden arm with an eye bolt in it. Bolt came loose, lost grip & fell back catching self with left hand.

Using a garden rake to pull clay balls from vibrator screen, rake hung in clay pulling on employee as the clay fell into the gob chute.

EE was working on dredge winch cable using a pry bar to loosen the cable by striking the cable. He lost control of the bar, causing him to strain his right arm and neck, also lost his footing. Using the wrong tool and poor positioning.

Uasing a rake/pry bar to pull a small piece of wood out from the rock box on the dredge. The piece of wood broke off, his arm slipped & he bumped elbow on rock box.

Getting cables to put together to pull pipe out of water. While putting clevis on pipe, pushed the clevis too hard and it gave way mashing left hand index finger in between clevis and pipe. The result is a broken finger.

Miner had tightened bolts in scalping screen and was using welder to weld nuts to keep them from loosening up. Slag came off of nut and fell down shirt, landing in navel area of abdomen causing burn.

The john boat floor was slippery as water had accumulated in the bottom at one side of the boat.

Employee was examining the air cylinder in the float cell. When he started to come down the ladder, he took a misstep on the ladder which turned his foot sideways causing his knee to pop.

Miner was welding on back plate for skirtboard of Plant Feed Belt when he began to have a headache, face began to tingle and pulse rate increased due to fumes from several welders being blown around due to frequent changes in the direction of the wind.

Cold weather made the pipe more brittle than normal. Typically the pipe can be cut to a certain point then the pipe can be broken with the equipment to finish the break. Under normal conditions the pipe could have been broken this way. Due to the cold weather making the pipe brittle the amount that was cut was not strong enough to keep from breaking on its own.

Employee was working a come-along overhead at the end of his reach.

Employee was using post hole digger without the safety bar between the motor and the post hole digger.

EE entered engine area to grease dredge service pump. When entering EE sliped and his left hand came in contact with dredge exhaust. EE grabbed dredge exhaust to brace his fall.

Drilling 4.5" hole in siding @ plant overhead. Once drill went thru it grabbed and turned bit into 3rd & 4th finger on left hand.

The employee was installing metal sheeting on the prep plant roof when he became ill from the heat - mild heat exhaustion was eventually diagnosed.

Slurry is entering the streambed at 3 locations approximately 1/4 mile downstream of the dam. The flow rate is estimated to be 600 to 800 gallons a minute. The flow is contained and being pumped back into slurry impoundment #5. MSHA has done a full investigation of the accident and has released the impoundment for work to resume as normal.

As the ee was climbing the ladder, the ladder began to tilt backwards. He was trying to get off the ladder but was unable to do so. This resulted in the employee falling 4 ft and contacting a concrete floor.

Adjusting valve on 20" cyclones. Trying to close the valve that is stuck open, using a pipe to close the valve.
