Fall to lower level, (Not Elsewhere Classified)

EE fell while putting away tools at end of shift. (Had been installing permanent handrail.) EE fell from first floor (steel floor 16' 9"" above ground floor - cement foundation); multiple injuries. Had removed his harness at end of shift. Fell through safety tape that was installed between temp. & perm. handrail. Died on 2/23/05 as result of injuries.

EE Got off the Trackhoe to help load the pipe into the Fusing Machine. His Foot got stuck in the mud causing him to lose his balance. Upon losing his balance, his left ankle rolled to the inside causing the injury

Draining pipe from screen bowl to froth feed and got spider bit on right hand.

Over-exertion in lifting objects

Employee strained his back unloading the cable from the pallet on the tailgate of the truck.

Contact with electrical current

EE was assisting B & G service technician with replacement of impeller & housing on #5 underflow booster pump. EE was standing 4' from service truck when lightning struck the CB antenna on truck knocking EE to ground.

He was climbing down the ladder of the WA 500 with wash hose in hand and slipped hitting his left knee on the step below.

Cleaning up mud & spillage around plant feed belt with skidsteer - put foot on bucket to exit the machine and right foot slipped and he fell over bucket - right knee hyper extended

Caught in, under or between a moving and a stationary object

Getting cables to put together to pull pipe out of water. While putting clevis on pipe, pushed the clevis too hard and it gave way mashing left hand index finger in between clevis and pipe. The result is a broken finger.

Employee slipped and fell on the deck of a dredge, bruising the right side of his lower back.

Struck against stationary object

Our employee was traveling in a ""jon"" boat out to a dredge. The boat had a small outboard motor attached to it. The throttle on the engine malfunctioned and the boat did not decelerate as anticipated. The boat then struck a ladder attached to the dredge. Upon impact, our employee fell forward, striking his knee on the deck.

Employee was trying to remove rusted cap on acetylene tank when he struck left thumb with hammer. Dr. diagnosed broken left thumb.

EE WAS CUTTING PIPE ON WINDY DAY. WIND BLEW DIRT INTO EE'S EYE. EE WAS WEARING SAFETY GLASSES WITH SIDE SHIELDS AND FACE SHIELD.

Struck by falling object

AN 18"" PIPE WAS BEING CUT ON PIPE STANDS. WHILE TURNING THE PIPE IN PIPE STANDS TO CUT BOTTOM SIDE OF PIPE, THE PIPE FELL ON EE'S RIGHT FOOT. PIPE STANDS ARE FOR 12"" PIPE MAX.
