Employee was dislodging elevator buckets in #2 jig elevator shaft and inadvertently stepped into the opened elevator access repair area underneath the plant jig. Employee was able to catch himself before falling completely into area and was lowered to plant floor below by two employees. Injuries consisted of sprained right knee and lacerations on left elbow and right shin.

MECHANIC WAS TRYING TO TAKE A TRACK APART AT THE MASTER LINK. THE MACHINE HAD A DEAD BATTERY AND WOULD NOT START. INSTEAD OF MOVING MACHINE UNTIL MASTER LINK WAS IN POSITION RECOMMENDED IN SERVICE MANUAL, THE MECHANIC STOOD ON TOP OF THE TRACK AND REMOVED THE BOLTS. WHEN HE REMOVED THE LAST BOLT THE TRACK CAME APART AND LAUNCHED HIM OFF OF THE MACHINE.

An unintentional roof fall occurred at the No 4 belt entry, inby spad #1364 one break. The fall measured approximately 30' x 19' x 8'. The roof fell on beltline and belt structure. The fall has been timbered off and will be cleaned up. The fall effected the passage of men but not ventilation. The area was supported with 5' fully grouted roof bolts.

Employee was standing on flat spot the operator was pulling the pump hose over where the employee could retie the rope on the pump hose, then the rock that his left was on rolled and the employee fell. When he tried to block his fall with his left hand he hit his hand on a rock dislocating the middle joint and tearing the skin.

This victim was utilizing a hammer & punch to drive a race off a bearing. The hammer deflected off the side of the punch and stuck his left thumb. Three stitches to close the wound.

Victim was removing the cutting edge from a 980 loader bucket. He was on next to the last bolt when the cutting edge swung down and struck him on the top of his left foot. He went to his family doctor and was sent for an X-ray which showed no broken bones, bruises only. Missed 3 days work to allow swelling to subside.

Victim was driving a powder truck up an incline. There was loose material on the roadway, which caused the truck to lose traction. The truck rolled back onto the berm and overturned onto the passenger side. Victim's safety glasses caused a laceration above right eye. Required 4 stitches to close.

Victim was checking for a leak on a rock truck. To determine location of the leak the bed had to be raised & lowered. He had another employee raise & lower the bed. He got his finger between the hoist & mast and when the bed was lowered it pinched the tip of his finger off. No bone involved. Cleaned & wrapped at hospital.
