Showing all 4EE WAS WORKING DAYSHIFT AT MINE OPERATING A LOADER(l220 VOLVO). EE EXITED LOADER, WALKED TO REAR, STOPPED,MADE ONE STEP BACKWARDS AND HIS RIGHT HIP FRACTURED AND HE FELL TO GROUND. THE WALKING, WORKING SURFACE WAS EVEN, CLEAN AND CLEAR OF ANY OBSTRUCTION. SUPERVISOR, NOTIFIED LOCAL AMBULANCE SERVICE TO COME TO LOCATION FOR CARE.
EE was loading up a trax running gear for a John Deere Dozer so it could be taken to shop for repairs. EE pinched the end of middle finger on left hand. The end of finger and fingernail was smashed. EE was transported to Providence Hospital for treatment.
EE was carrying a screen (4' by 6') up the stairs to the plant and strained a muscle in the middle of his back. He stated "I do not feel like I need to go to the doctor." He called in sick on the morning of 07-27-07 and stated he felt he needed to go to the doctor at this time to get his back checked. He worked all day 7-25-07 and 07-26-07.
