A T-Channel strap fell from the bolter and struck the employee below the left knee. The strike resulted in a laceration to his lower left leg.
A roof fall occurred on the Sub-Mains section (#13 Block, #23 Belt, #2 Entry). The fall area was approximately 16' wide and was bolted with 6' torque tension bolts and 12' cable bolts. Cribs were set at the fall location. No equipment, ventilation, or persons were affected.
A fall was discovered on E-1 Panel in the #1 Entry that was an alternate escapeway. The fall area measured 184' long and was bolted with 6' and 8' SuperTwist bolts and 16' CC cable bolts. No people, equipment, or ventilation was affected. The escapeway was re-routed.
During the weekly examination at this idled operation, five areas experienced a fall since the previous examination. Primary support bolts (8' torque tension bolts with resin) were pulled in the areas during the fall.
