Employee was installing a wishbone pin in a 777 when they felt a sharp pain in left shoulder. They stated it felt better as the day went on but throughout the night it started hurting and burning more.
Employee was welding cracks in a tube on a 777 Rock Truck when some grease that was in the tube got to hot and shot out of the tube burning our employee
B & W Resources Inc. believes an electrical short in the electric fuel hose reel caused the fire. The fuel hose reel compartment was severely damaged.
EE was operating an Ingersoll Rand drill near the outer edge of EE's drill bench when EE attempted to dismount the drill. As EE stepped off of the drill onto the berm, EE's footing gave way and EE fell. This resulted in a fracture to EE's left arm just above EE's wrist.
Employee was feeling tightness in the chest and was dizzy. EE was flown to the Medical Center for possible heart attack. It was determined by EE's personal physician that EE had a slight myocardial infraction. EE started missing work on 09/11/2024.
The employee strained EE's lower back while pulling an oil hose from the oil truck.
The employee was operating a John Deere 700 dozer when the machine over traveled the work area and overturned. The employee was wearing a seatbelt and only sustained bruising. EE was examined immediately after the accident and released for work later that morning.
The employee's left shoulder was injured when the left side of the loader bucket on the caterpillar 992G EE was operating caught a large rock. This caused the employee's upper body to twist to the left causing pain in EE's shoulder.

The employee was injured when a contract employee walked up from behind them, wrapped their arms around them and then squeezed them. The employee said they immediately felt like something was wrong. At the end of the shift they went to the doctor to be examined. Employee was diagnosed with a broken rib.
The employee was climbing onto a D9T dozer when the employee slipped on the wet track. As employee fell employee stuck right hand out to catch self and cut it on the back of the blade.
As the employee climbed onto the water truck, EE heard and felt a large pop in the right shoulder. This was immediately followed by intense pain and lack of movement.
It is believed that an oil inlet line ruptured or separated near the turbo causing the fire.
As the employee dismounted drill, employee slipped on the lower step and dropped to the ground. When this occurred the top step struck employee in the mouth and broke part of the left top front tooth off. The broken piece was found and reattached by employee's dentist later that day.
The employee fell while dismounting the 980H Caterpillar Loader. This resulted in a fracture to EE's pelvis and limited mobility in the shoulder.
The employee received a small laceration on the left side of EE's nose. This occurred after a small piece of metal broke loose from a pressing plate that EE was hammering on and struck EE in the nose.
The employee injured EE's back while dismounting a 992G loader. The injury appeared to be a muscle strain on EE's left side. EE refused initial medical attention but later visited EE's primary care physician when the soreness didn't subside. EE was treated and given a full release.
The employee claims that as EE was being loaded, a rock jarred EE's truck causing an injury to EE's neck.
EE experienced back pain as EE mounted a drill, stood up and turned to EE's right. This resulted in sharp pain originating in lower back and traveling down left leg.
While dumping material to reclaim a highwall, a Rock Truck driver got to close to the dozer running the dump and backed into the rear of the cab. This resulted in damage to the dozer's operated cab and caused the dozer operator to strain a muscle in EE's neck.
The employee was injured while observing the roll out of track for a Caterpillar D10R dozer. As the track unfolded a thin 1"x1" piece of metal broke loose from the track rail and struck the employee in the groin area. The employee was taken to the hospital for an examination and was placed on light duty.
The employee injured the small finger on right hand as employee was driving post for a silk curtain near the hollow fill pond. Injury resulted in 4 stitches.
The employee was tightening a hydraulic hose. The wrench slipped hitting the employee over the left eye. Injury resulted in 7 stitches.

The employee was working on realigning the tail pulley on a conveyor belt and twisted knee during the work process. The employee stated ee wasn't for sure if the accident occurred when ee got out of the truck or during the realignment process.
The employee was removing an oil filter. As the employee applied pressure to the oil filter in a twisting motion the oil filter broke loose causing pain in the employee's right shoulder. On July 12th, 2018 the employee was diagnosed with a torn rotator cuff and placed on light duty.
Sudden impact to both hands, resulting in numbness and pain.
When a 992G loader was cleaning the base of a highwall. The bucket of loader knocked a section of rock lose that fell onto the bucket. The loader was jolted around causing the employee to hit their right side on the arm rest. Seat belt was being used.
At 1:20pm EE was operating a Caterpillar 345C excavator when EE noticed black smoke. EE immediately exited the machine and seen large amounts of oil on the ground. This supports the idea that a major hydraulic hose blew causing the fire. EE and other company personnel attempted to extinguish the fire but were unsuccessful.
The employee was using a sledge hammer to assist in the installation of a ball stud on a Caterpillar 777D truck.
There was no accident, the employee was suffering from a bulging disc in which employee had previously had surgery for years ago. The condition was worsening and while working on the job that day employee was operating a 777D rock truck. As the employee stood up to exit the truck employee's back was burning and cramping.
The employee was cleaning loose material away from the highwall in the #8 coal pit when the bucket on the loader hit a rock. This caused the loader to abruptly swivel hard to the right sending a jolt through the employees back.
Truck driver was moving a fuel hose by kicking it and suffered a broken right foot.
Employee was kneeled down on the track of a Caterpillar D6N Dozer assembling the engine compartment. Employee twisted around to retrieve a panel for the engine compartment and pulled a muscle in employee's back.

Employee removed a hydraulic hose and got hydraulic fluid on EE's safety glasses and in EE's eye. After EE's eyes were flushed and cleaned, EE had blurred vision and was taken to an optometrist to be checked out. Blurred vision lasted for 2 days.
Employee was performing maintenance operations on the superior highwall miner. As he began tighten bolts on the hydraulic filter his Hitachi Impact caught fire causing 2nd degree burns to his hands.
Employee was attempting reposition a ceramic shoot for reassembly of the lid covers. The employee kicked the chute causing damage to the metal clamp that secured the shoot onto the piping that ran from over head. This allowed the chute to fall striking the employee on the right leg. Employee was examined at the local hospital and placed on light duty.
