Employees were working on replacing a bearing on the shaker. As they were removing the counterweight with a chain hoist, the bearing slipped off and smashed their right pinky finger.
Employee was putting the main boom back on the 300 excavator, lost EE's balance and fell forward off the excavator track and struck an adjustable step ladder striking EE's right shoulder and right ankle.

EE went home and then to the emergency room and had EE's finger and ankle X-rayed. It was determined EE broke EE's left ankle and dislocated EE's left pinkie finger

Employee was walking down steps from sand classifying plant and cut hand on handrail. Lacerated fingers, required three (3) sutures at medical facility.
