Caught in, under or between a moving and a stationary object

Employees were working on replacing a bearing on the shaker. As they were removing the counterweight with a chain hoist, the bearing slipped off and smashed their right pinky finger.

Employee was putting the main boom back on the 300 excavator, lost EE's balance and fell forward off the excavator track and struck an adjustable step ladder striking EE's right shoulder and right ankle.

Fall to the walkway or working surface

EE was on EE's knees under the big dryer baghouse assisting with bag removal. When EE went to stand up EE rolled EE's left ankle and fell. In doing so, EE injured EE's left pinkie finger. EE went home and then to the emergency room and had EE's finger and ankle X-rayed. It was determined EE broke EE's left ankle and dislocated EE's left pinkie finger

THE DREDGE CHAIN WAS STRUCK AND EMPLOYEE AND INJURED WAS PRYING ON THE CHAIN TO PUT IT BACK IT BACK INTO ITS TRACK. AS EE WAS PRYING ON IT INJURED WAS WATCHING IT TO MAKE SURE IT WAS GOING BAC K INTO ITS TRACK. THE PRYBAR SLIPPED AND STRUCK INJURED ON THE LEFT EYE EYEBROW.
