A brow in the #17 crosscut of the intake primary escapeway fell and rolled into the entry. The brow was 6' x 5' x 4' thick and impeded travel in the entry. Two (2) 4ft resin bolts were pulled out when the brow fell. After examination by a certified employee, floor to roof support, in the form of wooded cribs, were built to support the area and the rock was removed.

Employee was re-anchoring the shuttle car cable in the #4 entry between 203-204 crosscut on the MMU 002 working section when employee slipped, twisting and fracturing the right ankle.

Roof fall in 207/208 block, Number 2 entry. Fell on the Continuous Mining Machine when Miner operator was removing Miner from area. Miner had to retrieve with hydraulic retriever.

While riding out on the mantrip, the driver hit a crib causing employee to strike nose on the mantrip.

While sounding and scaling the roof, employee drew hand back quickly to avoid a rock that was dislodged straining right shoulder.

While operating a rubber tire mantrip the steering wheel jumped out of employee's hand. Tried to stop wheel by grabbing it. The wheel hit right index finger breaking it.

Fall in return discovered in #8 heading 1/2 block out by spad 2100. Operator immediately ceased mining activity when the fall was discovered. This was an unattended area of the mine.

#90 crosscut 3-2 and straight of #3 Roof Fall in Primary Escape way. The fall is approximately 70' long, 5' high, and 17' wide. All approaches to the fall area were immediately timbered, and the escape way was rerouted. The fall will not be cleaned up.

While mining in the number two entry Cont. Miner off side panic button malfunctioned. Not allowing the machine to move. While immobile, the inby pillar cut fell in to the turn posts. No rock fell on the miner. After making electrical repairs, the miner was able to remove itself from the cut.

A roof fall occurred out-by the 002 section on #11 belt between #208 and #209 x-cuts. The fall was 16 feet wide by 15 feet high by 45 feet long. The fall was reported because it was above anchorage. Approaches were re-bolted and cable bolted. Fall was cleaned up and a square set was installed according to our roof control plan.

A roof fall occurred in the 2 tol x-cut at #260 block outby the working section. Both sides of the x-cut had been cribbed and intersections were bolted with additional cable bolts. The x-cut was deemed bad long before it fell. It was cribbed off already. It will not be cleaned up.

#8 Belt tore apart. EE and others were repairing the belt. While cutting the belt to make a splice, the knife slipped out of EE's hand and cut his right lower thigh. The foreman, EMT, administered first aid, and EE went to ER to have it checked. He got 3 stitches, but the doctor said next time he did not have to come in for a cut like this.

While shoveling the #14 belt tailpiece, employee pulled something in the middle of his back. He finished the shift and it began to hurt worse on the ride outside.

On the 002 Section in the #2 entry face, a rock fell out of the roof striking EE while he was roof bolting and knocked him to the ground. The crew was able to remove the rock and transport him to the surface while performing CPR. He was taken by ambulance to Ruby Memorial Hospital where he was pronounced dead at 1130 pm.

On the 1 North Mains in the #4 face of the 002 Section, the continuous miner cut into an uncharted borehole which was an 8" pipe used as a power borehole to the former Humphrey mine. The pipe was sealed accordingly to plan and the area was made safe. There were no injuries.

A roof fall occurred in the #7 entry 1 block outby last open x-cut in an intersection. The fall was 18'x18'x9'high. No people or equipment was involved. All sides were cribbed and dangered off. The fall will not be cleaned up. We are now installing 6'/12' cable bolts in intersections until we reach x-cut #251.

Employee was running the miner making the initial cut in #2 to #1 x-cut. A small rock (less than 10 pounds) struck his hat then slid down his face cutting his cheek and lips on the left side of his face. The rock was very sharp. The area was fully roof bolted.

EE was positioning the drill station on tram side of bolter when a rock fell from between 2 rows of bolts. His ATRS was set and his canopy was up. He had his left hand on the edge of the drill steel tray when a small rock fell and struck his left hand. He was wearing metacarpal gloves.

Employee installed his 1 & 2 roof bolt on operator side. When he lowered his canopy, he started to clean rock off of it. A small piece of rock fell from the roof and cut his left forearm.

Two workers were pulling a cable off of buggy reel when EE felt a pop in his back. He did not begin to miss work until 1/21/13.
