Miner was changing segments on loader (Cat #4-11 988F) bucket when EE placed EE's palm on the carriage bolt head and applied the impact gun to the nut to remove. The bolt head spun and cut through the miner's glove and lacerated EE's hand.

Showing all 29Operator was mucking mud with excavator (Cat 365) when a hydraulic cylinder next to the cab burst. The window was shattered, and the operator was sprayed with hydraulic oil. Operator went to the hospital where EE was given a return to work without limits. EE chose to take the week off work. No injury, near miss only.
The plant operator was working with EE replacing a bearing at the head pulley of the sand stacker when EE cut EE's index finger on EE's right hand.
Miner was changing the teeth on an excavator bucket (Cat 390 #1-15). The miner was using a hammer to strike the retaining pin on the tooth when a piece of steel broke and hit the miner in EE's right leg causing a laceration.

Cat 775G Haul truck #3-74 dumped material into hopper and began to move away from dumping chute when fire started in engine compartment. Operator immediately exited machine without injury and fire was extinguished by nearby water truck.
At approx 2:39 PM under cloudy cool conditions, EE had just completed repairs to vibratory screen (S-9) and had descended the catwalk to the ground. EE was preparing to enter the passenger side of the repair truck cab when EE succumbed to a seizure. Paramedics were called immediately and removed EE to seek medical attention.
