Operator was mucking mud with excavator (Cat 365) when a hydraulic cylinder next to the cab burst. The window was shattered, and the operator was sprayed with hydraulic oil. Operator went to the hospital where EE was given a return to work without limits. EE chose to take the week off work. No injury, near miss only.

Loading/unloading boxes, bent over to pick up a box, pain in back.

Employee was in proximity of another employee welding when a piece of hot metal landed on shirt burning the employee's arm.

Employee was lying on left side and felt pain in their ribs. Dr. advised lt duty for 1 week.
