Employee was packing a piece of top belt structure, moved from right to left and felt pain in lower back. Employee stretched and finished shift.
Had a reportable roof fall at XC #2, Entry #3, on East Mains. Rock Fall does not have escapeways or airways blocked, it's in a neutral entry. Fall doesn't affect any people or equipment. Area has been dangered off.
EE stated gloves were wet from working on Equip. Put 1 glove under bucket on P/C, found can of soup from days past under second bucket, when moving soup off P/C, can burst in EE's right hand, lacerating middle & ring fingers & palm.
Had a non-immediately reportable rock fall in an Outby area of the Mine. The fall occurred on the Left Fork Panel, in the intersection's only of the #3 and #4 Entries, in Break #11.
EE was working in an unsafe position while changing out a reducer on a Scoop. While come-a-longing the reducer up, the reducer slid over and hit the side of EE's right foot. ***-- This report was submitted earlier under the wrong Mine ID, 1519945. This is the correct Mine ID and report. Thanks.
Had a non-immediately reportable rock fall in a panel. The fall happened on the Left Fork Panel, in crosscut #12, in #5 entry. Employees thought they heard something. Examiner went and checked and found fall. It was immediately dangered off, timbers set, till such time cribs could be built.
Continuous miner was cutting in #3 Entry, in crosscut #20. Had two pieces of rock on the continuous miner standoff, the bottom piece was the larger piece. EE was trying to remove the top piece of rock from the miner, when the top rock moved, both rocks came off together striking EE.
EE was stacking coal with a 980G loader. As EE backed up EE dropped off a pile of coal that EE missed that was 2 feet tall. EE's seat bottomed out and stubbed EE's lower back.
Installing roof bolts in the 3Left Cut, Break 12, Inby Spad #102. This was the first cut took out of 3Left, mined straight ahead. While drilling outside hole, in their first row, the corner on the operator's side fell off striking employee,on backside/left shoulder.
Injured was shoveling coal out of tail piece on main line (LW Mains) when EE felt a pain in the lower back.
Employee stated that they had cut a hole in the top of a hydraulic oil container to fill the continuous miner with oil. After emptying the container, employee stated that they threw the container to the side and cut their right index finger on a jagged edge of the container. Employee was taken to hospital and received two (2) stitches.
Employee reported that as they were helping to move miner cable, they felt a "pop" in their right shoulder. Employee missed no days of work. But they have ended up having to have surgery. Their surgery is scheduled for 07/23/2024.
Portion of the highwall above the #3 Portal fell. The rock landed on the canopy and the Stacker Belt Tail Piece. All employees were withdrawn from the mine. The area was dangered off until the proper agencies and company management was notified. No persons or equipment was injured or included in the fall.
EE was getting off dozer & put all weight on left foot to step down and it popped behind EE's left knee
Injured was drilling to install rib bolt and pulling drill steel from drill hole when a rock fell from top and struck EE and pinned EE against the head of the roof bolter. Injured suffered apparent fractured ribs and possible internal injures. EE was transported to local hospital.

EE was picking up a "fork" that had came off of the fork lift. As EE was positioning the fork to put it back on the fork lift, it fell, striking EE on top of the right foot. EE was taken to the local hospital and received three stitches.

EE states was loading the tray of the bolt machine with glue. As EE was loading the tray, EE dropped the box of glue. EE states that as EE bent over to pick up the box glue, EE felt a strain in the lower back.
EE was loading crib block into the bucket of a scoop by hand. As EE was loading the crib block, EE states that they felt a "pop" in right shoulder with immediate pain.

Employee was cutting tape off of chuck with a belt knife, and cut their hand in the process.
Employee climbed a ladder to check the feed box in the plant the ladder moved causing them to hit their right side ribs on the wash box. Employee then climbed down the ladder and reported the accident.
Employee was removing the lids from the top of the continuous miner, when their hand slipped, causing the lid to mash their left thumb.

Injured was loading bolt straps onto a lo-trac and as they lifted a heavy bundle of straps and turned to put on lo-trac, they felt a sharp pain in their lower back and pelvic area which began to swell immediately.
Area of roof unstable and working, was observed the area was dangered off with plans to re-bolts but before roof bolts machine could get to the area, a roof fall had occurred. MSHA notified and K order was issued.
Injured was changing tire on lo trac and as EE let the jack down, EE had their hand on top of tire. As jack dropped, the finder of lo trac caught EE's hand between finder and tire causing a laceration and bruising. 4 stitches were necessary to close laceration.
Section was pillar mining at the time with old works 140 feet above the section, and apparently there was a mountain break 140 ft up to the old mine workings and the mine was flooded releasing that water down and into our Putney Darby Mine # 1.
Pulled something in back adding rods on drill
Employee was servicing the miner setting bits when employee slipped on a muddy surface (incline) falling backwards onto right lower back. Employee stated back popped and pain started. The employee was transported to the local E.R.
EE stated that EE was cutting conduit with EE's knife when the knife slipped causing a laceration to the left index finger. The laceration resulted in 7 stitches. EE did not lose any time and was not duty was not restricted.
EE STATED THAT A ROCK WAS STUCK BETWEEN THE BLADE OF THE SCOOP AND BUCKET. EE WAS USING A 3 FEET LONG PRY BAR TO REMOVE THE ROCK. WHEN THE ROCK CAME LOOSE THE WEIGHT OF THE BLADE DROPPED ONTO THE BAR, CAUSING THE BAR TO PIN EE'S FINGER BETWEEN THE BOTTOM OF THE SCOOP BUCKET AND THE BAR. EE RETURNED TO WORK ON LIGHT DUTY.
EE STATED THAT EE WAS OPERATING THE SHUTTLE CAR, LEAVING THE MINER WHEN EE STRUCK EE'S HEAD ON THE CANOPY OF THE CAR CAUSING PAIN TO NECK AND BACK. EE CONTINUED TO WORK UNTIL 6:30 PM WHEN EE CAME OUTSIDE. EE SAW THE DR. ON 6-30-22. X-RAYS AND CT ARE NEGATIVE. EE IS NOW OFF WORK AND WAITING ON AN MRI TO BE PERFORMED FOR AN EVALUATION AND DIAGNOSIS.
While grinding / cutting a piece of 1/4" thick angle iron metal, the grinder jumped/kicked back and struck the left hand.
EE STATED: WAS TAKING A TIE OFF DOWN FOR THE SHUTTLE CAR CABLE WHEN THE TIE OFF DROPPED CAUSING A SUDDEN JERK TO EE'S ARM RESULTING IN PAIN AND BURNING IN THE RIGHT ELBOW AREA. EE WAS SEEN AT THE ER THEN HAD AN MRI ON 5-24-22.
A ROOF FALL WAS DISCOVERED AT APPROXIMATELY 10:58 AM IN THE NUMBER 1 ENTRY INBY SPAD 1617 IN THE 3RD LEFT PANEL BLEEDER. NO PERSONS NOR VENTILATION WERE AFFECTED. THE FALL IS APPX. 150 FEET LONG X 20 FEET WIDE X 5 FEET HIGH. THE FALL WILL NOT BE CLEANED UP AND A PLAN TO RELOCATE THE BMP'S HAS BEEN SUBMITTED TO MSHA.
EE STATED THAT A RIB ROLLED AND STRUCK EE'S BACK AND CAME TO REST ON EE'S ANKLE. EE WAS TRANSPORTED TO THE HOSPITAL WHERE XRAYS SHOWED A FRACTURE TO THE ANKLE.
EE stated was carrying metal roof straps to the front of the bolt machine when EE stumbled and fell. When EE fell, EE landed on the metal strap resulting in a laceration to the right forearm. EE was transported to Harlan ARH then transferred by air evac to Pikeville medical. A tourniquet was applied by air evac immediately prior to flight.
EE WAS GETTING A SHUTTLE CAR TIRE OUT OF A RAM CAR WHEN EE TWISTED A KNEE AND THE TIRE STRUCK RIGHT KNEE/ANKLE AND FOOT. EE CONTINUED TO WORK UNTIL 2-16-22 WHEN SURGERY WAS SCHEDULED FOR 4-11-22.
EE WAS ROOF BOLTING WHEN A PIECE OF DRAW ROCK FELL AND STRUCK RIGHT FOOT/ANKLE. EE SAW A DOCTOR AND WAS ALLOWED TO CONTINUE WORKING UNTIL 11-18-21 WHEN IT WAS DETERMINED THERE WAS A FRACTURE OF THE END OF THE FIBULA. EE BEGAN TO LOSE TIME ON THAT DATE AND IS CURRENTLY OFF WORK.
EE stated that EE twisted a knee as EE was turning to walk to the continuous miner on 10-5-21. EE continued to work and consulted a doctor. An MRI was done and EE had surgery on 4-7-22. This accident became a lost time accident on 4-7-22.
Miner picked up a 4lb hammer and EE's back hurt.
EE STATED THAT EE HAD FELT EE'S KNEE SHIFT TO THE LEFT WHILE PICKING UP BELT STRUCTURE IN MUD. EE STATED THAT EE FELT PAIN AND CONTINUED TO WORK. ON 9-20-21 EE SAW A DOCTOR THAT PUT EE ON LIGHT DUTY PENDING SCHEDULING AND RESULTS OF AN MRI.
When entering a lube truck cab to disengage the PTO pump. As they used the grab/handrail to pull themselves into the truck, their shoulder popped. Employee continued into the truck, but, when they got out of the truck shoulder started hurting.
EMPLOYEE WAS NEAR THE LOWER RIGHT RIB (CORNER) WHEN A PIECE OF ROCK FELL FROM THE RIB STRIKING EMPLOYEE IN THE LEFT LEG IN THE ANKLE AREA. EMPLOYEE WAS TRANSPORTED BY VEHICLE TO THE HOSPITAL. EMPLOYEE WAS DIAGNOSED WITH A BROKEN LEG.
EE was tramming roof bolter when EE ran over broken mine bottom & caused head to hit canopy of bolter. EE was placed on light duty until EE visits orthopedic dr. Further investigation revealed roof bolter was being pushed by scoop after getting stuck on slick, broken bottom. EE stated EE was in operators deck attempting to tram bolter & head struck canopy of bolter during process.
EE WAS ATTEMPTING TO DRIVE THE CAP BACK ON THE GEAR BOX OF THE MINER WITH A CRIB BLOCK WHEN THE CAP FELL OFF MASHING EE'S LEFT THUMB. EE WAS TREATED AT THE HOSPITAL FOR A BROKEN THUMB. EE RECIEVED STITCHES AND RETURNED ON LIGHT DUTY.
