During the removal of a sheave from a pump assembly, using a block of wood as a protective buffer while applying force with a sledgehammer. EE A was holding the block of wood against the sheave while EE B struck the block with the sledgehammer. During one of the swings, EE A attempted to reposition the block of wood, & the finger was pinched/smashed between the wood and the sheave.
Employee was tasked with installing a vulcanized splice in 17-6 conveyor belt. As part of the assigned task, employee tripped which caused them to fall forward and strike their mouth against the cooker. Contact resulted in a broken tooth and laceration to employee lip.
EE reported feeling a popping sensation in left wrist while using a pipe wrench. EE reported some tenderness in outer part of left wrist. No swelling or deformity noted.
Maintenance Technician (M1) was tasked with installing the track on a Bulldozer. M1 used a hammer to align the track master link. During master link alignment M1 felt flying debris enter their right eye. M1 was transported to the onsite clinic for evaluation and then transported to an offsite medical facility.
EE reports via phone that while working to replace the dump cables on the #3 dragline, when beating them out of their sockets and missed the swing. EE reports that this missed swing caused the movement to snatch the EE left's shoulder. EE reports left shoulder pain with movement.
EE reports walking to open valve to fill water tank, when right foot slipped on slippery ground causing right knee to twist while trying to maintain balance and not fall. EE describes right knee pain that prevents normal movement and prevents ability to work.
EE was exiting the truck to clean windows and mirror on R side of truck and when EE stepped on the lowest step the step gave way and the EE fell hitting back and bottom on the ground and head on the bottom front tire. EE seen at ER w/no restrictions. On 12/15/25 EE had lingering complications. On 12/17/25 diagnosed w/fracture of L shoulder, not able to RTW until reevaluation.
Fell into a washout pipe while connecting it, with a dislocation of left shoulder
Effected employee (EE) was helping load a grease barrel on a fuel truck when another employee was on top of the truck pulling the top of the barrel causing the bottom of it to slide out and pinch EE's left thumb under the barrel and deck of the truck. The barrel was 16 gallons and weight approx. 120 lbs. Weather conditions were gloomy and misting rain.
Operator in Training (O1) was tasked with wash down duties on the north side of the Mill Washer Building. While performing the assigned task O1 used both hands to close a 2" ball valve with an oval handle. While closing the valve, O1 felt a pop in their left ring finger. O1 was evaluated at an offsite facility, where imaging revealed a chipped bone in the finger.
EE reports that EE was walking in the mine area and stepped on a spot that moved under EE's foot causing foot to twist. EE reports pain in foot and unable to walk due to pain. EE states that EE has recent problem with foot, but that the current pain is much worse.
EE reports low back pain that began while working on a pump this morning. EE describes middle of the back pain that is keeping EE from being able to move or bend in any direction. EE states unable to work with the pain. EE reports previous low back problems that normally resolve with prescriptions and rest.
EE reports pulling on a wrench and experiencing a pop in the right-side chest muscle. EE states pain with movement of arm. On physical exam there is a tender lump in the muscle.
EE while in the process of removing debris/sticks from sleeve of pump, EE tripped over a plate that is part of the sled and fell backwards into a washout landing on left side. EE reports neck, shoulder, mid & lower back pain with stiffness. EE reports previous back injury.
EE reports walking between truck and dragline and turning right ankle by stepping on uneven ground. EE states that EE reported it immediately and had applied ice to the area without relief. EE was brought to the clinic by supervisor. EE unable to walk normally due to pain and swelling to inside and outside of ankle.
EE was aligning strap with suction flex when left thumb got caught and crushed between the strap and suction flex as it was being lifted.
EE reports getting off the #3 dragline on the off-cab shoe ladder. When stepping off ladder left ankle rolled. EE unable to bear weight and had notable swelling in ankle. EE was sent to ER for evaluation by supervisor.
Employee was arc gouging bulkhead weld seam off Matrix pump sled. While cutting the last of the weld seam the wall shifted pinching a finger between the floor and head of the gouging torch. Employee received laceration to right second finger. Employee was transported to Nutrien site first aid office and then ER and returned to work on 08/22/25.
Employee was welding and tacking troughs in Enviroclear tank. EE took a break, 5-6 minutes, kneeling in front of fan to drink gatorade. When EE went to stand up, EE passed out.
EE reports a popping sensation & a sharp pain in right side after slipping in mud & nearly falling. EE denies fall explaining that it took effort to catch balance. EE completed shift & reported to the Clinic by phone on 7/14/25. Follow-up call on 7/15/25 morning = no improvement. EE reports that the pain has not improved with rest & states unable to do much because of the pain.
EE's right hand was caught between the belts & the sheave while in the process of changing drive belts on final product pump. EE had to roll sheave in the opposite direction to remove the stuck hand. EE visited Clinic for after-hours treatment & returned to Clinic on 7/14/25, had decreased grip strength, swelling to thumb, ring & little finger. Hand was obviously painful to move.
EE stated that while in the process of backing the haul truck to empty it into the #2 pit, the rear of the haul truck entered the pit causing the cab of the unit to overturn on the passenger's side. EE was able to self-extricate from the haul truck. EE was observed by co-workers to have a laceration on head and swollen right shoulder area. EE reported pain in both areas.
EE called Clinic to report neck strain that occurred while pulling hose, prevented return to work the next morning. EE reports stiffness in neck that prevents turning of head left or right. EE was instructed to visit the Clinic for evaluation by company MD. EE evaluated and referred to clinic for evaluation and treatment.
Nutrien Employee was in the process of loading a mines power cable PLM head into the reel of the reel loader. The cable snagged, causing NE to drop the PLM head. The head fell onto NE's right ring finger pinching it against the reel.
EE began experiencing pain in right shoulder while working on the control cards on the #5 dragline. EE states the repetitive movement & the work to get the cards in and out of the slots caused the problem with the shoulder as the work was difficult & repetitive. EE states decreased range of motion due to pain. Denies previous problems with shoulder & is unable to work due to this.
EE Reports left shoulder pain from excessive vibration and movement in the heavy equipment that has become worse since. EE reports pain with decreased range of motion in the left shoulder.
EE reportedly stepped into a small hole of missing grating causing a twisting fall to the decking floor. EE landed flat on back. EE found by ERT standing holding onto desk for support in control room. EE stated back pain that prevents normal walking. EE refused ambulance transport but did request transport to ER for evaluation.
EE reported left wrist pain from grabbing handrail during a near slip and fall. EE states that the area is very wet and slippery. EE describes popping sensation in left wrist at time of incident, and states that the pain has increased since the incident.
EE reported left knee pain after working in the dragline tub replacing tub paper. EE states that the knee was hurting too bad to straighten all the way and that it hurts too much to bear weight on it. Upon presentation in clinic, there was notable swelling to the front of the left knee.
EE was taking apart a lube line and twisted wrist. After a few minutes EE reported pain in wrist and was checked out by EMT on shift.
Equipment operator stated EE wasn't feeling good and asked to go home. Operator missed two days the week prior with the flu. After operator badge out and left active mine area to personal vehicle area the operator entered EE's vehicle at approximately 7:51 and was found diseased in EE's vehicle approximately 6:15 p.m.
EE called and reported left knee pain while getting on and off a D8 Bulldozer. EE states that the incident happened on Friday night and that the pain has increased since becoming bad enough that it prevented EE from being able to work Sunday & Monday night.
EE states stepped down into the mud on the way to the pit car. EE states that left foot went into mud and boot became stuck. EE states left ankle and left knee twisted and has pain in both. States hurts too bad to work on it. Denies previous injury or problem with left foot or knee. Left ankle with notable swelling on examination.
Employee alleges that EE strained EE's lower back while performing repairs on an excavator. EE did not notify management until 2/10/2025. EE states that EE was handling a started when the event occurred.
EE reported pop sensation in lower back during climb into boom truck. EE states left shop and drove to work area before realizing that low back was too painful to allow standing up straight.
EE reports packing #9 lift on the #2/4 matrix line closing the matrix bleed-off valve to release the line back into operation when low back pain was experienced. EE reported the injury the next morning by calling as the EE was unable to get dressed to return to work. EE requested off-site evaluation and was sent to Company Care for evaluation and treatment.
Mill Maintenance Mechanic (M1) bent down on their right knee to observe the rotary gate valve at the #6 calciner. M1 began feeling pain in their right knee as they attempted to stand back up from the kneeling position. M1 was transported to the onsite clinic and was later transferred to an offsite facility for additional evaluation.
EE reports discovering that right foot was swollen and painful after work the day before. Reports slipping on catwalk while working on pit pumps at 5-pit at about 13:30pm on 12/22/24. The EE only noticed the injury upon arriving home after work.
Employee reports feeling discomfort in right knee after getting down from truck.
EE called Clinic on 11/18/2024 to report getting a blister from new work boots on 11/14/2024. EE missed work on 11/15/2024 and went to personal doctor. PMD treating wound for infection and provided prescription antibiotics. Followed-up with PMD and received instructions to remain off work and follow-up with podiatrist.
EE reports something pop in right heel followed by pain with a burning sensation while in the process of climbing onto a dozer in the process of completing pre-shift inspection.
EE reports feeling a popping sensation in the right knee upon stepping down from bulldozer in the process of hooking up a cable to the pit pump. EE presents to Clinic with notable antalgic gait. EE unable to fully straighten leg due to pain. EE states remote history of injury to same knee.
Maintenance tech. was in the process of replacing an air cannon @ the #2 gate east side. M1 had a ratchet in left hand and a 15/16 wrench in their right. The ratchet slipped off the bolt and struck M1 on the bottom lip. M1 immediately reported to the supervisor & went to the first aid clinic. M1 was carried to an offsite medical facility and received three dissolving sutures.
EE reports right knee pain after twisting into position getting in the CAT 980-wheel loader. EE reports that the knee has not improved with ibuprofen and ice and there is additional swelling.
EE was working from the bed of a truck and hopped down to the ground. EE experienced pain in right knee with swelling. EE states that the pain is increased with pressure and motion. Restrictions were given that cannot be accommodated. EE is off work until further notice.
EE was the front seat passenger in the Com veh leading another to a job site when the driver of this veh stopped to look at a deer. EE driving the 2nd veh struck the back of the 1st veh despite stopping. EE reports neck & back pain. EE also reports history of previous back problems. Restrictions were given that cannot be accommodated. EE is off work until further notice.
EE was driving a Company vehicle that was leading another to a job site and stopped in the road to look at a deer. EE driving the second vehicle didn't see that the front vehicle stopped and struck the back of the stopped vehicle. EE reports stiffness in upper back.
Mine conditions were favorable with no adverse weather conditions.
