Quarry Superintendent and Operator were in the lower lot area of the quarry inspecting for a place to dig and haul sand with the excavator and haul truck. As they were walking the area inspecting for a spot to dig, operator stepped into a rut and rolled left foot and fell to the ground.

Showing all 6Employee was carrying a rectangular shaped wear plate that weighs approximately 30-40 pounds. Employee attempted to step up onto an elevated platform and lost EE's balance which cause EE to stumble. As a result, the plate smashed EE's right hand between the wear plate and a cross bar member resulting in a broken right index finger.
Crew was excavating a pipe out of a trench w/a loader. Employee A was on the loader being directed by Employee B on the ground. Employee B directed Employee A to pull forward when they did the pipe wedged between the teeth on the bucket. This caused the pipe to rotate & struck Employee B in the shoulder & head. No broken bones concussions or contusions. Just sore.
Employee was picking up a boulder to widen the path for a haul truck to access the crusher hopper. After picking up the boulder, employee hit finger on the haul truck and busted finger open.
Employee was stepping off the ladder of the loader and rolled ankle on a rock.
Tech was replacing a bearing race on conveyor tail shaft. Tech used a die grinder to cut the race and then used a hammer and chisel to wedge apart the race to drive off shaft. A metal piece of the bearing race or chisel chipped off and embedded in the side of the hand. The small (approx. 3cm) metal piece had to be surgically removed at the doctor.
