An employee was using a 3" high pressure water hose to fill an equipment pressure washer and the employee lost control of the water hose and it slapped the employee in the left shoulder and chest, causing a strain to the upper left arm tendon, shoulder muscle and chest.

Showing all 10Scale Master, slipped and fell, walking down aggregate and dirt embankment. They fractured the right side of their left ankle and caused a hematoma on the left side of their left knee.
Fell on right shoulder, bruised.
3 inch rock bounced off shaker screen and hit operator in the mid back causing a small bruise.
