Based on investigation it is belived that EE climbed up on top of the pan feeder on an extec crusher while it was in operation. This was in violation of company policy & training. EE went through the crusher and was killed.

Struck by flying object

An employee was using a 3"" high pressure water hose to fill an equipment pressure washer and the employee lost control of the water hose and it slapped the employee in the left shoulder and chest, causing a strain to the upper left arm tendon, shoulder muscle and chest.
