Bad safety link on safety chain, securing manlift basket to manlift.

Fall to the walkway or working surface

Employee was carrying package product from the truck to the bore holes. The work area was extremely muddy and slippery from several days of rain. The employee slipped and fell injuring a hip. EE saw a Dr. and was put on light duty

Crusher operator was grinding on a piece of metal and a chip of metal got into eye. The operator had safety glasses on, but they didn't have side shields on them.
