While attempting to remove the strap holding the roller/bearing, the weight shifted and caused the roller and bearing to shift and struck the Techs right hand causing a laceration to EE's (R) index finger, requiring 7 stitches.
The HVAC vent covers in the floor of the office building/ lab were removed for floor cleaning and waxing. Mill operator was exiting the lab area when EE stepped on said opening, making EE loose EE's balance, leading to a fall. Employee was transported to hospital via local ambulance service. Employee was seen by hospital ED for pain in knee. Xray's indicated no broken bones.
Employee was relocating a waterline pipe in the shop to be fabricated and pinched EE's pinky finger between the pipe and floor when setting the pipe down. Employee was taken to the urgent care to find a fractured fingered.
Operator walking, overstepped end of EE's catwalk, causing EE's toe to catch the ground, causing EE to fall forward to the ground, landing on right shoulder.
While holding the belt the injured worker re positioned a hand to secure the belt from moving, the worker cutting the belt continued cutting the blade moved & cut the left hand behind the index finger. The injured worker did have their PPE Cut Glove on during the incident.
The employee was assisting in loading an X-ray machine into the back seat of a pickup truck. The weight of the machine shifted in EE's hands unexpectedly, resulting in EE's hand being pinched between the corner of the machine and the center B pillar/door jam of the vehicle, resulting in a laceration to the right ring finger.
During the time of the incident the temps were in the mid 20's and it had snowed earlier in the day, resulting in slippery track conditions
Hearing loss
Lab tech was in the laboratory, walking into the mill testing area from the main room. When transitioning through the doorway from the main room into the mill testing area, the miner slipped on the tile walking surface causing the miner to fall to the floor.
Two maintenance employees were adding a return roller and brackets to BC-124. While putting the roller into the brackets one side went in with ease. That employee then went to help EE's partner by lifting the belt. The struggling employee hit the roller with EE's had causing it to fall out of the bracket and land on the side of EE's boot fracturing EE's pinky toe
While attempting to loosen an air hose on a rail car with a 24" pipe wrench FLW overexerted self and irritated an existing condition. Incident resulted in a sprain/ strain injury to the right arm requiring modified duty work restrictions. Follow-up is required.
Employee was stepping over a wash hose and caught the heel of EE's boot causing EE to fall onto EE's left knee.
While lifting the lump basket from the loading opening of a bulk pneumatic trailer FLW experienced a "pop" in the center of the back. Continued working until about 10:00 AM until they determined they would require medical attention. Diagnosed with a "lumbar back strain".
Stress of repetitive motion
Miner injured middle finger of right hand between tailgate and bed of a dump truck.
While attempting to pry against a stuck rotary mill, an employee propped a large pry bar in the inspection port, while positioning to look into the hole, the bar slipped out and fell onto right thumb causing a contusion and a fracture.
Maintenance Mechanic working on a bearing to a fan completed the repairs and was re-installing a pulley guard, the guard got stuck and as the Mechanic pulley on the guard to free it to complete the installation EE's (R) ring finger and pinky finger got pinched between the guard and the fan. Causing a fracture to the tip of EE's ring finger.
While repairing a damaged trunion tire, the employee needed to trim a piece of rubber in order to get the puller installed. The box knife being used slipped lacerating the employee's left ring finger.
Employee has a confirmed STS in EE's left ear that has results in a loss of hearing >25dB
Employee was going to help clean at the top of the penthouse. While walking to the area one of the plugs for a clean out hole was laying on the catwalk and the employee tripped over the plug and hit their knee on the expanded metal flooring.
Employee was attaching a trailer mounted pump to a track hoe bucket in order to move the pump into position. The track hoe bucket unexpectantly lowered, catching the employee's hand between the bucket and pump frame.
While removing bolts from haul truck cab, employees finger was smashed between wrench handle and frame of truck causing a small laceration on left pinkie finger requiring 4 stitches. Employee put on modified work duty until stitches can be removed in 7-10 days. No bending, pushing, pulling, or repetitive movements with 5th finger until healed or sutures are removed.
EE was adjusting the weight on a vibrator in the dryer burner room when EE's foot slipped a little causing EE to put all EE's weight on right shoulder causing EE to dislocate right shoulder. EE was taken to the ER where the shoulder was treated and EE came back to work the next day.
Fluid bed sand dryer was plugged with wet sand. Operator was checking the progress of unplugging efforts by opening an access door to the discharge chute. Operator did not stand aside when opening the door and was struck by hot sand. Injuries sustained were first and second degree burns to face, head, neck and upper arms.
Mechanic moving a classifier from its housing when it got hung on a vertical I-beam on the lifting eyehole as it twisted coming out of the housing. Mechanic placed their hand on the motor to push away from the beam. Space was too tight for a pushstick. The lifting chain shifted as twisted and caught right thumb between the motor and chain. 6 stitches.
On 9/16/24 at 7:00pm an employee fell while getting off the dozer when re-fueling. When EE caught self from the fall, EE injured right elbow. EE came in to work the next morning at 8:15 and was brought to urgent care by the PM. After X-rays and other checks, it was found that EE had a right radial head fracture.
Employee was opening bags with knife and cut EE's shin
Dryer operator attempting to pull water hose, slipped on algae that had grown on concrete surface, causing operator to partially fall, catching self with right hand/wrist. X-ray of hand/wrist showed a sprain that required a brace and restricted work duty for 10-12 days. Operator was placed off work until the x-ray was read 8/5-8/7
Shop employee was accidentally struck in the face with a shop hammer by another employee while trying to remove a pin from a dozer track. Employee received 3 stitches.
Employee stepped wrong and felt a pain in left knee while walking from truck to pump MCC.
Employee is reporting blood clots associated with sitting in a excavator. This claim is being contested by the site.
Two employees were moving a tote of coag using a pallet jack. One employee was pulling on the pallet jack handle while the other was pushing the tote. The employee pushing on the tote felt a pop in their left calf muscle.
Strain on groin doing mine moveup. Doctor put on restricted duty same day.
Replacing Filter Belt and felt discomfort in right forearm by EE's elbow. Doctor put on restricted duty starting on 06/20/2024.
Miss stepped going down stairs causing right knee injury. Doctor put EE on restricted duty on 06/21/2024.
Carrying a boom lock for 824L wheel loader tripped on something on ground & dropped boom lock on right hand pinky finger
Maintenance employee cut the back of left hand on a piece of sharp metal while placing a cooler in the back of the service truck.
While treating mine water at Outfall 2, hydrated lime in the hopper escaped through a gap at the flow adjustment plate and hit the employee in the eyes. Employee immediately began flushing eyes with bottled water. Due to severe irritation, the employee was taken in for medical evaluation.
While attempting to remove a stuck bolt, employees hand slipped from tool causing EE's left forearm to strike a piece of plate steel, causing ~2" laceration. Required 6 stitches.
Repetitive motion carpal tunnel symptoms were reported to Management on 04/23/2024. Had surgery on 08/09/2024.
Reportable hearing loss
While attempting to remove a screw conveyor coupler, via striking, employee suffered a lumbar strain.
Employee was using a grinder and slipped and lost balance while grinding. Employee received an injury to right index finger which required 3 stitches.
Employee was cleaning scales with a skid steer when EE hit the gap in the panels with the bucket causing the machine to stop suddenly, jarring the employee forward causing EE's head to hit the cab.
Employee was walking up stairs when pain was felt in the lower back/right hip area and it was reported to the supervisor that evening. The employee took the following day off 3/6/24 and visited the clinic where they recommended work restrictions. The employee returned to work on modified duty 3/7/24. The employee returned to work with no restrictions 3/12/24.
Pulled muscle while shoveling.
On Saturday morning at around 6:15 am EE was trouble shooting the burner at the dryer. After several trips in and out of the burner room EE, tripped and stumbled. While falling EE's hardhat came off. EE fell on left knee and striking head on a metal cabinet causing a laceration on EE's scalp resulting in five staples. DR returned employee to full duty.
Employee was putting a return roller into place on conveyor, and it kept falling out of place. Employee kept catching it with their left hand over their head and then felt pain in their left shoulder.
A customer truck pulled into the plant. While waiting on instruction to be loaded a fire started under the cab of the truck. An employee used a fire extinguisher to try and put out the incipient fire. The wind blew smoke & dry fire extinguisher powder toward the employee which the employee inhaled. A local fire department put out the fire which burned for approximately 40 minutes.
