EE was walking up the belt line when he slipped and hurt his right shoulder trying to stop his fall.
EE was unloading cable from the back of a man trip. The mantrip drove over a bundle of roof mesh. When trip got on top of the bundle it spun out and set the top piece flying into employee's right ankle causing it to roll over.
EE was walking down the face towards the head gate with the shear, he tried to step over the anti slew device at shield 100 and his feet became tangled causing him to trip and fall twisting his lower back.
The tailgate of the longwall section closed off due to caprock falling in.
The employee was setting a timber using a jack pot. The timber broke and hit him, and the jack pot hit him in the head.
Employee was trying to install a DA shear pin back into the DA advancing bar on the longwall shield. While trying to install the pin a face bounce occurred and threw the bar up into the cable tray with his finger in the middle.
While drilling holes to anchor the section tail piece, the air line came loose from the drill and sprayed air and debris into individuals eyes, the foreign material in his eyes needed to be removed at the hospital and the Optometrist office.
Employee was driving a Gehl loader into the section. He stopped at the phone to call into the section. He turned the Gehl off and was lifting the safety bar on the machine, when it rolled backwards and hit the rib.
Employee was in the pan line putting the shear cable back into the brettbe. A face bounce occurred and a chunk of coal came away from the face area and pinned his leg to the sigma section of the panline breaking his leg.
EE was standing in the panline on the longwall face at shield #123 insalling the shear cable into the brettbe when a bounce occurred throwing the employee's leg into the spill tray. EE was seen in the ER and released to return to work. His knee continued to bother him for months after and he had his knee scoped on 1/3/2007.
The employee was shoveling coal fines away from the rib when he slipped on the wet floor and hurt his back.
Carrying a tow jack down to the longwall to put it in the back of a truck, he walked through a brattice curtain and couldn't see the rock prop on the other side, which caused him to smash his small right finger between the rock prop and the tow jack lacerating his finger open.
While re-installing the relay bar pin to the pan line. A crib block was used to apply pressure to the relay bar, assisting in aligning the holes. The crib block flipped out and the shield rocked forward striking the individual's hand, causing a fracture to his right little finger.
Exposure to Hydrogen Sulfide Gas to the eyes
Raising monorail to hang on chains from the roof. EE rested the monorail on his head.
Sweeping floor, during movement, struck a metal file cabinet, which caused laceration to right index finger.
EMPLOYEE WAS HANDLING STEEL, MOVING & SORTING, WHEN A PIECE CAUGHT HIS GLOVE ON A SHARP EDGE, CUTTING HIS SMALL FINGER ON HIS RIGHT HAND.
The employee was cutting a "flapper" off the belt. The flapper was approx. 60' long and 14" wide. He needed to cut it into shorter lengths so he could drag it out of the structure. He was cutting towards himself and when the knife slipped it cut his thigh. He required 3 stitches.
The employee was bending over getting dressed in the bath house and felt a sharp pain in his lower back.
While walking down the longwall set-up face, individual struck head against a roof bolt hanger. This jammed his neck. On 3/27/06 individual visited a doctor, the evaluation found his neck to be in spasm and causing a mis-alignment. Individual was kept off work and received physical therapy.
The shearer was cutting from the head to tail gate. The shearer was cutting to the bottom rock. At shield #109 the shearer operator saw flame on the headgate side of the shearer. The flames were extinguished immediately.
While mining a lift off of a pillar, the rib sloughed off some chunks of coal and one struck individual in right ankle and foot. This caused a fracture to right foot and a severe sprain to the ankle.
While moving a water pump into water hole, stepped on rock and twisted left knee, did not see a doctor until 3/16/06 for treatment. Treatment will be for strained or torn ligaments with physical therapy and knee brace for support and comfort.
EE was attempting to pull an empty 5-gallon hydraulic can out of the frozen ground. As he was pulling on the handle of the can, one end of the handle broke loose from the can and cut his little finger (6 stitches required). EE was wearing a pair of insulated leather gloves at the time of the incident.
The Shearer operators had cut a taper from #40 shield towards the headgate. The headgate drum was just a foot or two from cutting through the headgate when a bounce occurred. The bouce blew coal through the bounce shields striking him
The employee bent down to pick up and move the miner cable. He felt a sharp pain in his lower back and couldn't move.
EE was assisting the section electrician make a cable splice when his razor knife slipped and cut his left thumb. EE was taken to ER and recived 3 stitches.
Afternoon Shift Shut belts off at about 3:20 to replace a bad roller. After the roller was replaced the belts were restarted and the beltmen checked belts as they were starting, all started down to # 16. Beltman went into # 16 drive and tried to start the belt but it would not start. A beltman went to the tail to check for a plug and found the roof fall (4:20 pm)
A bounce occured that disrupted ventilation. The face fan was displaced from the force of the bounce and had to be moved and ventilation re-established.
EE was attempting to cut a hole in the center of a piece of belting when the razor knife that he was using slipped and cut his left knee.
While releasing the roof screen mesh from the top of the ATRS individual had right elbow pinched between mesh support plate and ATRS main frame. This caused a severe bruise to right elbow area, resulting in missing work to allow swelling to subside.
While installing a timber in the breaker row, individual felt pain in lower back. Individual went to Chiropractor and found to have a back strain. He is receiving treatment for this and has not been released to return to work.
The Employee was standing by the pager phone on the headgate relaying information off the face to the surface. A bounce occurred on the pillar just outby the headgate controller. The impact of the bounce blew a ?rock prop? on him striking his left foot.
EE was dropping a bolter cable from the mine roof, as he unhooked the cable and it fell to the ground the j-hook snagged the EE's hand causing a laceration on his right hand.
After EE ran shear for the shift, he felt a slight irritation in his eyes. The next morning when he woke up, his eyes were swollen shut. EE went to his eye doctor and was examined and given 2 presciptions.
The employee was dropping miner cable off of the hangers. As he lifted the cable off the hangers, he slipped and fell back onto his elbow.
EE was pounding a wedge in between a timber and the mine roof when a piece of debris got in his right eye. ee was wearing safety glasses but was working in a strong air current.
The Employee was cleaning out for a timber (outby work area). A small piece of cap rock fell from the roof and struck him on the wrist. The rock cut his wrist and required 5 stitches.
While installing a water hose to the section water manifold, the manifold broke free from the water line and struck individual in the left shoulder and side of face. This chipped his bottom left wisdom tooth and caused a hair line fracture to jaw.
EMPLOYEE WAS ATTEMPTING TO RETRIEVE A STUCK DRILL STEEL. AS HE PULLED ON THE STEEL IT RELEASED, SMASHING HIS RING FINGER BETWEEN THE BOTTOM OF THE DRILL STEEL & THE DRILL BOX ON THE BOLTER.
The continuous miner operator was attempting to make a cut. The coal seam had a rock spar running vertically through the face from 3 to 15 inches thick. A flame occured about mid cut and traveled to the floor. The flame only lasted about 15 seconds.
Employee was finishing the repairs on the continuous miner. He lifted the cover to recover tools and/or parts. The cover slipped out of his right hand and fell onto his left hand. The cover smashed his left thumb causing a laceration and fracture. Employee sustained a fracture of his left thumb and required 5 stitches.
EMPLOYEE WAS STRUCK IN THE BACK & SHOVED FACE FIRST INTO A SHIELD. THERE WAS A BOUNCE ON THE FACE.
While changing bits on the longwall shear, a small piece of rock fell from the face striking individual on the right hand causing a laceration to ring finger that needed 6 stitches to close.
The employee was walking. He slipped and fell on a wet rock. He attempted to catch himself and injured his right shoulder.
EE was assisting the miner operator in clearing some cap rock off the head of the miner. As they pushed, the cap rock broke and EE's hand slid along the edge of the rock. The sharp edge cut through EE's gloves and cut his left thumb. EE was transported to the hospital where he received 5 stitches.
Coworker got Employee's attention & indicated that he had stuck a roof bolt. Employee hurried over to the left (cab) side of the bolter and tried to save it. He ran the head up while holding the roof bolt wrench. When the wrench came in contact with the bolt, because the resin had already set, the bolt/wrench kicked in against the jaws, pinching employee's finger.
While cutting thru the tailgate a piece of coal bounced off the face and hit employee in the head causing a laceration which took 6 stitches to close.
Employee was attempting to remove 7 roof screens from a bundle of screens that were leaning against the rib when the entire bundle tipped over striking ee in the left leg.
The miner was taking a point in the pillar section. Top rock fell on the miner and interupted normal prodution for over an hour.
