Struck against a moving object

THE VICTIM WAS OPERATING DIESEL TRACTOR PULLING A WATER TRAILER. HE DROVE TO THE SURFACE AREA AND PARKED THE TRAILER. HE THEN PULLED AWAY FROM THE TRAILER AND RAN INTO A LONGWALL SHIELD WHICH WAS STORED ON THE SURFACE AREA OF THE MINE.

Struck by powered moving object

IT APPEARS THAT THE VICTIM GOT OUT OF THE TRUCK WHILE IT WAS BEING LOADED. THE TRUCK BEGAN TO ROLL FORWARD DOWN AN APPROXIMATE 6% GRADE ALONG THE MAIN HAULAGE ROAD. THE TRUCK TRAVELED ABOUT 200 FEET, LEAVING THE ROAD, AND HITTING AN EMBANKMENT. THE VICTIM WAS FOUND ON THE GROUND ABOUT A FOOT AWAY FROM THE REAR OF THE TRAILER.

Absorption of radiations, caustics, toxic and noxious substances

Employee was sitting at his job station within the Guard Station at the main gate when he was bit in the armpit area on his right arm, by a spider. Employee worked the remainder of his shift & then was transported to the local hospital where he received medical treatment for an allergic reaction from the bit.

While walking through the shop, individual tripped over jack handle and fell onto right knee, treated for contusion to right knee.

Individual was helping guide the movement of 988 loader when the loader bucket contacted a roof jack that was lying on the ground. The roof jack flipped up and a section of the roof struck individual in the side of his hard hat causing a ruptured disc in neck.

Over-exertion in lifting objects

The employee bent down to pick up and move the miner cable. He felt a sharp pain in his lower back and couldn't move.

Caught in, under or between a moving and a stationary object

Employee was finishing the repairs on the continuous miner. He lifted the cover to recover tools and/or parts. The cover slipped out of his right hand and fell onto his left hand. The cover smashed his left thumb causing a laceration and fracture. Employee sustained a fracture of his left thumb and required 5 stitches.

Coworker got Employee's attention & indicated that he had stuck a roof bolt. Employee hurried over to the left (cab) side of the bolter and tried to save it. He ran the head up while holding the roof bolt wrench. When the wrench came in contact with the bolt, because the resin had already set, the bolt/wrench kicked in against the jaws, pinching employee's finger.

The miner was taking a point in the pillar section. Top rock fell on the miner and interupted normal prodution for over an hour.

Caught in, under or between a moving and a stationary object

The Employee was attempting to bend a roofbolt to put in the roof, the bolt slipped forcing his hand into the bolting machine. this resulted in a broken bone, in his left hand, behind the ring finger.

Accident type, without injuries

The supervisor of the miner section found a roof fall that was above the anchor point of the roof bolts in an ""ouby"" area of the mine. The fall was approx. 15' wide, 30' long and approx. 4' high. The roof bolts used in this area were 42"" mechanical that were installed 20+ years ago. The fall did NOT impede travel or ventilation.

Accident type, without injuries

The weekly examiner found a roof fall in the lower bleeder entry at approx. #60 x-cut. The fall is approx. 80 feet long, the width of the entry (19') and up to 6' high. The roof bolts in this area were installed 20+ years ago and are 42"" mechanical. The roof fall impedes travel and prevents the weekly examination of the seals inby.

Struck by flying object

The employee was performing a pre-shift examination for the oncoming Graveyard shift. He was walking through the Last Open Crosscut between the #2 and #3 entries. A bounce occured which blew coal out and struck him in the face and shoulder.

Accident type, without injuries

The pre-shift examiner found a roof fall along the pillar extending from the main entry to 1st right (later falling to 2nd right). The fall extended above the 42 inch mechanical bolts. The area was timbered and dangered off.

THE INJURED EE WAS INSTALLING A TRANSMISSION FROM AN ISUZU P.U. TRUCK LOCATED IN THE SURFACE SHOP. AS HE WAS LIFTING THE TRANSMISSION IN PLACE, IT SLIPPED AND FELL AND PINCHED HIS RT. HAND BET WEEN THE TRANSMISSION AND THE CONCRETE FLOOR. THE LITTLE FINGER OF THE RT. HAND RECEIVED A COMPOUND FRACTURE AND DISLOCATION. AS A RESULT OF THEINJURY FIRST JOINT OF LITTLE FINGER AMPUTATED.

Struck by rolling or sliding object

A SHUTTLE CAR WAS BACKING AWAY FROM THE CONTINUOUS MINER. THE FRONT OF THE CAR HIT A ROCK THAT WAS LYING ON THE FLOOR. THE ROCK SLID INTO THE INJURED EE'S FOOT CAUSING LACERATIONS THAT REQUIRE D SUTURES.

Struck by falling object

THE INJURED EMPLOYEE WAS TRAMMING THE ROOF BOLTER INTO A NEWLY CUT FACE. HE PARKED THE BOLTER AND WAS CLIMBING OUT OF THE CAB WHEN A ROCK FELL FROM BETWEEN THE BOLTS AND MATS STRIKING HIM ON THE HEAD. HE COMPLETED THE SHIFT BUT WENT TO THE DOCTOR AFTER THE SHIFT AND WAS DIAGNOSED WITH A SPRAIN / STRAIN TO THE NECK.

INJURED WAS OPENING A MAN DOOR. HIS RIGHT HAND WAS PINCHED BETWEEN THE DOOR AND THE DOOR FRAME.
