I.NAME WAS TROUBLESHOOTING A POSSIBLE BRAKE PROBLEM ON 777 TRUCK. I.NAME INSTRUCTED OPERATOR TO MOVE VERY SLOWLY IN FIRST GEAR WHILE I.NAME & MECHANIC FELT BRAKE ASSEMBLY. THE WHEEL RIM HAD A FLAT SPOT ABOUT 2 FT. LONG, WHEN IT ROLLED AROUND TO THAT SPOT IT PINCHED & CUT I.NAMES ARM BETWEEN RIM & REAR SUSPENSION CYLINDER, CAUSING INJURY.
DEFECTIVE BLASTING CAP WENT OFF CAUSED STEMMING TO BE BLOWN INTO FACE ETC OF BLASTER LOADING HOLE.
ROCK FELL FROM HIGHWALL WHILE EMPL WAS INSTALLING WATER PUMP HITTING EMPL IN BACK BRUISING BACK AND RIBS.
DRIVER STARTED UP PIT RAMP WITH A LOAD NEW HALF-WAY POINT TRUCK STALLED & STGARTED DOWN RAMP BACKWARD TRUCKS LEFT REAR TIRES CAUGHT BERM & CLIMBED UP IT CAUSING TRUCK TO ROLL TO RT SIDE THIS A CTION THREW OPERATOR TO PASSENGER SIDE & CAUSEDBRUISED AND BUMPS TO BOTH ARMS AND HIS BACK IT ALSO CAUSED LACERATION NEAR RT EYE
