Operator was drilling a hole with a TD230 bolting machine. A piece of drawrock fell onto the ATRS, when he lowered ATRS, the rock slid off the machine, scratching his back. Also, the drill steel fell against his neck, buring his neck.

EE had just finished installing timbers and began to observe the top for the miner operator who had begun to cut the end cut in the #2 entry. A section of roof measuring 18' wide 0-60" thick fell on ee causing fatal injuries.
