While cleaning the off-side walkway on #4 belt, 2 Heintzmann jacks became dislodged and a large rock struck employee pushing him into the belt line. The accident is currently under investigation by the WVOMHS&T, MSHA and the Company.

ON JANUARY 20, 1998, AN UNEXPECTED ROOF FALL OCCURRED WITHOUT WARNING AT THE NO. 3 ENTRY ON THE 3-RIGHT SECTION OF THE LIGHTFOOT NO. 2 MINE. A FATAL INJURY AND A LOST TIME INJURY RESULTED FROM THIS OCCURRENCE.
