A Roof fall in 3rd South Panel entry 6 xc30 intersection area was bolted with 8 foot TR and 12 foot cable bolts and straps. Area had a slip in it and top was adverse condition, ventilation was not affected, stoppings were intact and area is dangered off on all sides. Roof fall will not be cleaned up.

A roof fall occurred on 3rd south panel, entry 4, xc 21. The fall occurred in a fault zone. The fall area was rebolted and cleaned up using the miner and scar. Additional cable bolts were installed on all sides of the fall as well as the placement of cribs and timbers. Mine ventilation was not effected. The fall was approx. 20' high and involved the entire intersection.

At 5:50 am on 10/16/2015 while checking the water level we found a roof fall in #2 entry (return) at 72 break. The fall was 30' long by 20' wide and 36" thick. There was 36" roof bolts and they came down with the fall. This is an untraveled area.

Scoop operator was hauling supplies to the section and the roof bolter cable got hung on the canopy of the scoop, operator tried to move the cable and his finger got smashed between the canopy of the scoop and the cable.

Employee was installing a roof bolt when the employee got his right index finger pinched between the roof bolt plate and the mine roof. Employee did not become a lost time accident until he had surgery on his right index finger on 10-29-08.

A roof fall occurred on the No. 1 section in the number 3 entry cross cut at spad 7135. The roof fall measured approximately 30 feet long, 30 feet wide, and approximately 9 feet in height. The roof fall fell on the No.2 MBC, No. 3 MBC,and portion of the belt tailpiece. The No.2 MBC operator was trapped inside the operator compartment for a brief time span but suffered no injuries.

The employee was helping make a belt splice on the #1 belt, when the belt splicing tool got jammed. As the employee was helping to free up the belt splicing tool, his left hand got caught, causing a fracture to his left thumb.

A methane gas ignition occurred on the #1 section at approximately 7:30 am. The continuous miner had cut approximately 20 feet out of the face area in the #3 left cross cut and was cutting in the top when the methane ignition occurred.

Employee was helping remove a bad pump motor on the miner, the chain hoist that was being used to remove the bad pump motor broke loose and hit the employee on the left knee.
