TWO SHUTTLE CARS COLLIDED IN AN INTERSECTION DUE TO A FAILURE TO COMMUNICATE BETWEEN OPERATORS AND THE PRESENCE OF A LINE CURTAIN WHICH HINDERED VISIBILITY. THE BUMPER OF ONE CAR ENTERED THE O PERATORS COMPARTMENT OF THE INJURED MANS CAR, SQUEEZING HIM AND CAUSING A FRACTURE TO HIS PELVIS.

Showing all 16RUN OFF OF WATER FROM THE MINE SURFACE AREA ENTERED THE MINE DRIFTS DURING A THUNDERSTORM. THE WATER PARTIALLY BLOCKED THE INTAKE ESCAPEWAY. THE ENTRIES HAD BEEN DEVELOPED BETWEEN 80 AND 150 FEET FROM THE HIGHWALL. THE WATER WAS PUMPED FROM THE MINE. NO INJURIES OR DAMAGE TO EQUIPMENT OCCURRED.
WHILE TRYING TO CLEAR ROCK FROM A COAL SHAKER HE WAS STRUCK ON THE RIGHT HAND BY A FALLING ROCK.
EE WAS SHOVELING COAL FROM UNDERNEATH BELT STRUCTURE & STRAINED HIS LEFT SHOULDER.
A ROOF FALL WAS DISCOVERED IN #1 ENTRY OF THE SECOND LEFT SECTION 25' OUTBY SURVEY STATION #590. IT MEASURES APPROX. 40'X 48'.
A ROOF FALL WAS DISCOVERED IN #9 ENTRY & X-CUT #8-9 AT STATION #591 IN THE 2ND LEFT SECTION RETURN. IT IS APPROXIMATELY 40' LONG & 48" HIGH.
A ROOF FALL WAS DISCOVERED BETWEEN #1 & #2 ROOM OFF 1ST L;EFT SECTION, TWO XCUTS INBY STATION #405. IT WAS ~50' LONG AND 48" HIGH.
THE MIHE EXAMINER DISCOVERED THE FALL ON HIS WEEKLY RUN. THE FALL DOES NOT EFFECT VENTILATION OR TRAVEL SO IT WILL NOT BE CLEANED UP. IT HAS BEEN DANGERED OFF WITH WARNING SIGNS.
EMPLOYEE WAS ASSISTING IN ADVANCEMENT OF BELT STRUCTURE WHEN ANOTHER EMPLOYEE WAS HAMMERING THE STRUCTURE AND A PIECE OF METAL FLEW AND STRUCK EMPLOYEE IN THE EYE.
A ROOF FALL OCCURRED IN THE INTAKE DRIFT, APPROXIMATELY 50' INBY THE HIGHWALL. THE AREA WAS SUPPORTED WITH METAL J CHANNELS ON WOODEN LEGS AND AT LEAST ONE 4' RESIN BOLT IN EACH J CHANNEL.
A ROOF FALL MEASURING APPROX 20'W X 30'L X 4'H OCCURRED IN THE #9 ENTRY OF 2ND LEFT APPROX 30' OUTBY STA 514. THE AREA WAS SUPPORTED WITH 48" AND 60" RESIN BOLTS AND ROOF MATS.
A ROOF FALL MEASURING 20' L X 20'W X 4'H, OCCURRED IN THE #4 ENTRY OF 1ST LEFT SECTION AT SURVEY STATION 397. THE FALL WHICH IS IN AN INACTIVE AREA OF THE MINE WAS DISCOVERED BY THE MINE EXAMI ONER ON HIS WEEKLY RUN. THE FALL DOES NOT AFFECT TRAVEL OR VENTILATION AND WILL NOT BE CLEANED UP.
EE WAS WALKING UNDERGROUND. HE STEPPED ON AN UNEVEN SURFACE, SLIPPED TWISTING HIS LEFT KNEE. A SHORT TIME LATER HE KNELT DOWN TO PICK UP SOMETHING, HIS KNEE POPPED AND HE COULD NOT STRAIGHTEN IT.
EE WAS ON THE SURFACE USING A SHOVEL TO CLEAN OFF STACKER BELT, WHILE SHOVELING HE TWISTED HIS BODY CAUSING INJURY TO HIMSELF THAT RESULTED IN A HERNIA. EE CONTINUED TO WORK DOING REQULAR DUTI ES TIL HIS SURGERY WAS SCHEDULED. FIRST DAY OFF WORK WAS JAN 22,01.
WHILE GOING FROM A KNEELING TO STANDING POSITION, EMPLOYEE PLACED HIS HAND ON THE BELT GUARD AND CAUGHT HIS FINGER IN THE MOTOR FAN THUS SEVERING RING FINGER TO 2ND KNUCKLE ONHIS RIGHT HAND. THERE WAS A HOLE IN THE GUARDING WHERE HIS FINGER PROTRUDED THRU ACCIDENTLY.
EMPLOYEE WAS OPERATING THE SCOOP WHEN HE BUMPED HIS KNEE AGAINST THE MINE ROOF. HE SUFFERED ONLY A MINOR ABRASION BUT SEVERE SWELLING RESULTED. HE HAD SURGERY ON THIS KNEE IN SEPT. 1988 AND HA S HAD PROBLEMS EVER SINCE.
