Fabick CAT employee was working in the Cleveland Cliffs Mining Shop. The employee was working on a motor grader cylinder head, installing a fitting. The wrench slipped off and the employee's hand hit a flange, on the cylinder head, causing a small laceration on the top of EE's left hand.

Employee was removing valve from Tilden 1 #2 South deaerator line when their left hand was pinched between the valve and a handrail. The employee was wearing gloves at the time of incident and sustained a laceration to their left hand. Employee was transferred to local ED for evaluation and received medical treatment (7 sutures) and returned to work.

Employee was removing a fire water valve when the employee's left finger was pinched between the valve and the hand railing while lowering the valve with a chain hoist. Employee was transferred to local ED for imaging which revealed nondisplaced fracture to employee's left pinky finger. Employee was provided a splint for immobilization and protection and returned to work.

EE was cutting piece of steel with 21" torch out of the stilling well in 4 Tails Hydro. EE cut from the bottom of seam leaving ~6" gap in middle, then cut seam from top down. The piece of steel shifted, broke off, and fell backwards over top of the rake making contact with EE left arm and face shield resulting in stitches to the nose.

Employee was using a pry bar when working on a bus, removing the transmission. The bar slipped and struck EE in the mouth, causing the tooth filling to fall out and broke part of the tooth.

Technician has lower left side back pain that radiates to the hip that started after the 993K engine tune-up technician was trying to install the engine turn tool. the technician's body is in a awkward position with head tilted and arms up against the engine block while on a 2 step ladder. After trying first aid, EE finally went to the clinic on 2-9-26.

EE entered into Otis freight elevator to get to another floor in the Pellet Plant while performing operational rounds. Elevator stopped operating. While on the phone with the MSHA OneCall, elevator technician arrived onsite & EE was able to exit the elevator. No injury to employee.

An employee experienced an incident while using a ratchet that became increasingly difficult to turn. During this process, the employee felt a pop in their right bicep. Following the incident, the employee was evaluated and was returned to work within their job class. However, the employee is now scheduled for surgical repair, which triggered reportability.

EE was exiting a Broderson mobile crane and caught their foot on the bottom edge of the Broderson doorway. The EE tripped and fell approximately 18-20 inches striking their left knee on the ground. The fall caused a minor scrape to their left knee, at a follow up appointment on 1/6/2026 the diagnosis changed to a closed nondisplaced transverse fracture of left patella.

Employee arrived to work for the start of day shift. On their way into the truck shop from their personal vehicle, the employee slipped and fell on ice in the parking lot and broke their hip. Employee was transported to the hospital for medical treatment.

Employee was making a shovel cable connection when the cable rolled, pulling the employee's left arm forward. The employee experienced left arm pain and was evaluated at the local ED where images revealed an injury to their left bicep. The employee was prescribed work restrictions and is being accommodated.

Employee was hosing in the crusher bowl when the employee tripped and fell landing on their left knee. Employee continued to work and felt something running down their leg when they observed a cut below the left knee. Employee was evaluated at the ER and received medical treatment (stitches).

MMG's were removing the 5B rougher drum in the east side of the Concentrator to repair the tub. They used OH crane to remove drum to set the magnetic rougher drum on ball mill floor cover. The cover became stuck to bottom of drum. MMG jumped on 5ft pry bar placed between cover & drum (@ angle ~4-5" from ground), when cover released, EE injured right foot.

The employee fractured the right thumb when it got pinched between two cables while using the crane and wrecking ball to scale down the west car dump pocket. The cable got hung up on a protruding stud and the employee attempted to move cable. The employee was wearing leather gloves at the time.

A fire occurred on a Komatsu 930E Surface Mining truck. The area was secured to prevent harm from potential hazards for the duration of the exposure. External fire response agencies were immediately contacted and extinguished the fire when conditions allowed.

Employee was pushing a 55 gallon grease drum with a drum cart when the tire struck a bolt in the grating causing the employee to hit their ribs on the cart handle. The employee was evaluated on June 23rd and returned to work without restrictions. The employee was seen again on June 27th and was then placed on restrictions as a result of the injury.

While attempting to catch the cover the employee's right middle finger went under the seesaw mechanism causing a fracture to the tip of the right middle finger.

While installing the #8 Support shaft, the safety holdback rigging was in place and not utilized. This allowed the support shaft to slip and pinch the employee's calf between the shaft and shaft support wheels. Communication between the crew and foreman was poor. Crew complacency decreased awareness

While rotating a haul truck final drive during re-assembly per service manual instructions, two employees were using long pry bars to rotate hub assembly. One of the employee's bar slipped out of its position on the hub causing the employee to fall backwards to the ground, rolling EE's right ankle causing a fracture to the employee's right tibia and fibula.

An employee attempted to throw a 3 pound rail anchor into the back of a grapple truck, but the rail anchor ricocheted off the sideboard of the truck instead of landing in the back and struck another employee standing nearby in the mouth, lacerating the employee's upper lip and fracturing the employee's upper jaw.

A hose released pressure causing an abrasion to the left cheek. Employee said they were loosening and wiggling the hose and valve connection when it made a pop and they felt the oil hit to their cheek. Spray of oil happened and only lasted a split second.
