The blade slipped out of flooring and cut into left wrist which was holding flooring up to cut flooring easier. 911 was called and employee was transported to hospital where they received stitches in left wrist as well as prescribed 600mg of Motrin and 500mg of Tylenol.

Employee traversing across plant site, encountered wet material and slipped rolling ankle. Employee was taken to the clinic where a diagnosis was made of a sprained ankle and fractured fibula.

Employee was cutting liner material with a utility blade, the blade slipped from the cutting path and cut the employee's hand.

After performing the annual hearing exams, the employee was designated as having threshold shift that required MSHA notification.

Third party aggregate hauler was exiting vehicle after being weighed, when the hauler slipped on the step of the hauler's truck injuring right knee. The individual's employer transported the hauler to the hospital where the hauler received surgery on knee.

Employee was changing screens on the wet shaker when the rail fell on EE's right hand injuring EE's middle finger. Employee went to clinic for treatment for possible fracture. Employee was found to have a BAC of 0.04.

Scrapped right shin on the basket of a manlift

The mechanic was attempting to reinstall a lift cylinder on an articulating dump truck in the parking area at Holliday Rock Jobsite. The strap that was attached to the crane and the cylinder slipped causing the cylinder to fall and strike the mechanic on the right-side causing accident to ribs and right arm.

Wet roads, and employee lost control of haul truck and ran into side of embankment.

Hosing down a conveyor belt when EE lost footing and strained the left ankle.

EE was opening the valve of a water tank when EE strained EE's right shoulder.

EE was walking down stairs when EE slipped and landed on EE's right knee.

Employee was loosening the bolt for a turn buckle of a motor to release the tension to replace the v-belt. When loosening the bolt, the turn buckle fell from its position crushing the tip of employee's left hand pinky finger crushing it between the turn buckle and the platform.

Employee was walking down a stairwell and lost their footing missing the step EE intended to hit and landing on the next step with their right foot spraining the ankle.

EE lifted the hood to replace a welding rod and a foreign object landed in EE's right eye.

EE hit hand on a piece of angle iron cutting left hand.

Employee hit hand/wrist on a steel plate while working on a cone crusher.

EE was climbing down a ladder after working on a shaker screen when EE felt pain in EE's left bicep.

EE was in the employee parking lot after finishing EE's shift and 2 co-workers noticed that EE was acting confused and suddenly fell to the ground. 1 worker took EE's shirt off and put it under EE's head, they noticed EE was going into some sort of seizure so the other worker called 911, the paramedics arrived in a few minutes.

EE says lower back started to hurt while shoveling

Miner was found unresponsive in cab of pickup truck.

While shoveling by a shaker screen a rock came off and hit miner in the left shoulder

Miner claims that after months of resting elbow on front end loaders arm rest it has caused a pain in left elbow. Claims the arm rest is too hard.

While monitoring the pit line, employee stood up and turned swinging hand into roller, employee's hand made contact with troughing roller. Roller pulled arm in up to elbow. Employee pulled arm out on own.

While cleaning out material chute employee stepped down and rolled left ankle.

While getting ready to cut a piece of angle iron (10' X 3") the angle iron slipped off the vice and struck left pinky finger.

Was dislodging a rock with pry bar, rock dislodged and pushed bar and hit hands on feeder box.

While attempting to dislodge rock from feeder, the rock shifted and pushed the pry bar into the handrail pinching left pinky finger between bar and rail.

Employee was attempting to cut a hole in conveyor belt, using a box cutter type knife. Employee accidentally cut the tip of employee's right middle finger.

Miner was exiting employee's wheel loader when employee slipped. Employee grabbed the hand rail with right hand. Injured employee's right shoulder.

Miner was using pry bar to free up a chain on a sprocket. When miner pulled back on bar it slipped off the sprocket, hit a steel plate pinching finger between bar and plate.

Stepping down off of tractor onto level ground twisted left ankle.

While removing cutting edges off of Loader WL-65, cut left middle finger on sharp bolt. Received stitches from doctor.

Walking toward rock plant control tower with lunch box and water bottle in hand. Tripped on concrete slab and fell to EE's knees.

Injured right forearm while shoveling.

While welding a hot piece of metal fell onto his shoe. Burned through his shoe and burned the top of his left foot.
