Employee was working under 2D stone hopper, breaking up lumps to get material flowing with a poke bar. A large chunk broke free and struck the bar, causing the end of the bar to strike employee's chin. Employee was taken to physician, received 7 stitches under chin and returned to work with no restrictions - Medical treatment only.

Lab tech was taking out trash at the end of the shift. When EE threw a 2 gallon bucket of clinker(<20 lbs.) in the side door of an eight yard dumpster, EE felt a pull in the left side of EE's stomach. EE was taken to the ER for medical evaluation, placed off work until a follow up with regular physician, when EE was placed on restricted duty. 1 day LT, 10 day RD

Employee locked out disconnect, removed guard and was removing belts from a dust collector fan by rolling them off the sheave. As was rolling a belt from the sheave EE caught glove. When EE jerked their hand back by reflex, found the belt had sheared the tip of left index finger off. EE was taken to physician, had the tip sewn up, and placed on restricted duty.

Employee was using a standard metal sledge hammer to remove the teeth off of a loader bucket when a piece of the tooth fractured and struck the employee in the arm. The piece of metal hit the employees left forearm causing a laceration and the metal piece to stick in the arm. The metal piece that fractured was approximately 1/4" (long) X 1/8" (wide) X 1/16" (thick).

Employee was attempting to adjust a fan damper when the ladder they were on slipped and they fell to the ground. As the ladder came to a stop at the bottom the employee jumped to the right and felt a pain in both ankles.

Employee was working in the #1 kiln dust collector re-bagging. EE noticed arms becoming irritated so put on a long sleeve shirt to keep dust off. The following morning reported arms at the bend of the elbow were showing signs of irritation.

Employee injured when boom lift overturned. Employee was coming down for break when EE drove the lift too close to sloped area causing the lift to overturn. EE sustained serious cut to facial area and other unknown trauma.

Employee was working in the finish mill building cleaning out a blockage in FM1 step chute for approximately 10 hours using a jackhammer. Jackhammer caused dust to stir from the material they were working on. The employee had cleaned self off several times and changed clothes twice throughout the day. Employee noticed the burn the next am before work.

Employee was unchoking silo 27 and got cement dust in eye. Flushed eyes thoroughly at the plant and was taken to physician where employee was given a anti-biotic ointment as a precautionary measure. Employee returned to work with no restrictions. Medical treatment only.

Employee was fueling an air compressor when employee slipped off of the second step of a folding ladder, landing on left hip. Rainy conditions may have contributed to the incident. Employee was taken to physician diagnosed with a bruised hip and possible lumbar strain placed on restricted duty.

Employee was working on a pickup truck in the maintenance shop, helping another mechanic to lift the bed off of a 1/2 ton pickup truck. Employee stated that ee "felt a "pop" in lower back but felt like it would go away". The next morning the employee reported the incident to supervisor and that back felt stiff.

A choke occurred in a kiln discharge chute. The injured employee was seen walking unexpectedly into the area housing the discharge end of the pipe. The employee later observed the injured employee was covered in dust material. Further evaluation revealed the injured employee had burns on hands, feet, torso and legs. The injured employee was transported to the hospital.

Employee was closing the clinker feeder belt guillotine. When the employee had finished; Employee began walking on the grating walkway to exit the area when employee struck knee against the conveyor belt. The employee got a superficial wound on knee that needed 2 stiches as treatment.

Employee was clearing lumps from a rotary feeder. In order to have manual control of the feeder the linkage was disconnected, when the employee reconnected the linkage ee caught left thumb in the clevis, causing a laceration. EE received stitches to close the wound, and was placed on restricted duty

Employee tripped and fell while measuring a silo. When EE lifted the cover, the silo was blowing dust out. EE turned head quickly to get out of the dust and tripped. EE fell forward onto the silo lid that is a 3'X3' box 18" off of the ground. EE's shoulder hit the lid first then the right side of face.

Employee was sampling cement from the air slide on FM 2 with safety glasses, hardhat and dust respirator on when there was a pulse of material through the air slide. Material flew out of the port and into the face of the employee, getting into right eye. EE immediately washed eye out and went to the ER for treatment.

Two employees were making a lifting for engaging an electrical engine; they lifted using the ball Eye ring and the reducer part of the engine. One employee put hand under the suspended load (engine) to install the screw at its base when The Eye ball ring came off the engine making the engine to get down and catching right hand fingers ( middle and ring fingers )

Electrician was checking grounding and rotation on a submersible pump. When employee jogged the pump, the reaction caused the pump to start to fall. When employee reached to stabilize pump, pinched hand between the pump and the discharge pipe. Employee received a hairline fracture to a bone in hand, and was placed on restricted duty

Employee was using a chain fall to rotate a plate on a ball mill. EE had put the hook on bolt & used left hand to hold the hook in place. As ee put a strain on the chain fall, the hook slid up bolt, pinching palm between the bolt head and the hook. Employee was taken to a phys. who cleaned and dressed wound, and placed employee on restricted duty

Employee was unchoking a vessel, when material fell down on the inside striking the bar EE was holding. The bar kicked up striking the nail of EE's little finger. Employee reported incident and was taken to OMP where a subungal hematoma was drained and an antibiotic was prescribed. Employee returned to work with no restrictions

Employee was traveling down an interior stairwell, when EE missed a step and rolled ankle. Employee was taken to Physician for medical evaluation, diagnosed w/ a sprained ankle placed on restricted duty. EE was released to full duty 7 days later. Stairs were clean, dry, well lit, and in good condition. Not a contributing factor. Employee stated EE just missed a step.

EE was welding in the reject bin when a piece of slag fell behind EE's safety glasses and landed on the inner corner of EE's left eye resulting in a first degree burn. Antibiotics and eye drops were prescribed. No lost time.

Employee reported incident to supervisor, was taken to OMP for medical evaluation, and placed on restricted duty (no lift, push, pull>10lbs, no prolonged overhead work

Employee was walking down a flight of stairs wearing cleated over boots when EE lost their footing. The stairs were made of a perforated grating for grip. The cleat caught in the grating and EE's momentum caused EE to lose footing. EE hit the bottom but said was ok. EE was sent to the clinic as a precaution. EE was given a 25 lifting restriction until 2/2/2015.

Employee was attempting to seat a valve on a diverting plate by striking it with a hammer. EE missed intended target and EE's hand struck an adjacent piece of metal lacerating the pinky finger requiring 3 stitches.

It is believed that a malfunction occurred within the coal mill when a plow broke. This likely promoted ignition of fire due to sparking. No injuries reported, filters and support cages damaged during fire. All safety interlocks functioned correctly as expected.

During a clinker breaker choke, employee climbed fixed ladder to check load on cooler. When EE stepped down, he stumbled & fell against railing, hyperextending his little finger. Was checked by physician-possibly flexor tendon rupture. Regular duty initially, expect restricted duty after surgery-2 days restricted prior to surgery, 8 days lost after surgery, RTW w/restrictions.

Employee was walking beside a railing, looking up when he struck his knee on the mid rail. Did not think it was significant and continued to work. Reported the incident the next day and was sent to Dr. for medical evaluation- placed on restricted duty. Worked 2 days & knee retained fluid, causing employee to miss 3 days work. Employee back to work under restrictions- doing well now

Employee was operating a 988F end loader. The air suspension compressor for the operators seat had a blown fuse. The repetitive bouncing through normal operation of the loader caused lower back pain.
