High winds caused the EE to miss bottom step while climbing stairs to EE's office. Causing injury to EE's knee.

Caught in, under or between running or meshing objects

The employee was sucking out the dust collector at Tower 2. The employee had placed the hose inside the hopper. The hose became clogged with a large chunk of clinker. The employee brought the hose part way out of the hopper to remove the chunk. The employee tried to remove the chunk from the end of the hose, and lost control of the chunk. His hand went into the rotary valve.

Struck by falling object

Employee was tapping cement and a lump got stuck in the valve at the bottom of the silo. The lump went through the valve in to the rotary screw causing the screw to jump violently. As the screw jumped, it cause a small piece of pipe that was resting on top of the screw cover to bound down on the employees LT foot.

Fall to the walkway or working surface

Employee was leaving the Change House at Plant 3 at the start of his shift. As he stepped outside the double doors of the Change House lunch room, he slipped on a patch of ice. He cut is left hand and suffered a hairline fracture of his RT hand.

Fall from machine

The employee was standing on top of a cover to the outside incline drag conveyor in order to remove the top section of the head pulley guard to #2 drag conveyor. The employee received a call on the plant radio, and when he reached for his microphone, he lost his balance, falling to the ground. He was standing 3' 10"" off the ground when he fell.

Employee was placing caution tape by #5 finish mill for a project. The employee was using a utility knife to cut the tape. While cutting the caution tape, the employee lacerated his left middle finger.

Employee was assisting with a conveyor belt replacement. He was removing an idler roll from the conveyor. As he was removing the roller, the far end of the roller dropped down and his thumb got pinched between the roller and conveyor frame.

Over-exertion in lifting objects

Employee was operating the bag applicator at the pack house. Empty bags were getting stuck in the feeder. The worker estimated that empty bags got stuck in the feeder about 18 times in a short period of time. The worker had stacked approximate 30 empty bags on the table. When he picked up the stack of bags, he felt a sharp pain in his left groin. First day off work was 6/18/14.

Employee was in the basement of the Pack house emptying a trash can full of shredded bags and cement weighing approximately 50 lbs. The employee grabbed the can handle with his left hand and placed his right hand under the can. As he lifted the can to about shoulder height, he felt his left arm and shoulder go numb.

The worker was removing lumps out of silo 25 by manually operating the silo valve handles. While rocking the handles back and forth to break up a lump, the valve jammed and the employee felt a sharp pain in his shoulder.

Employee was performing maintenance on the #3 finish mill. He stated while cleaning the area on the floor below the mill, he stepped on a hard lump of material that was buried under the dust, twisting his left knee.

Employee was using a jack hammer at tower 2 near the transition between 31 and 105 belt. The jackhammer blade got stuck in some wire mesh. When employee went to pull it free, the 2"" spade landed on his left foot.

Employee was working in #5 finish mill removing head liners. Four (4) liners are removed at a time. Employee had placed a liner at an angle to the side of the mill. While moving another liner, the one he previously positioned slid down striking him in the RT ankle.

After passing a jack hammer under a conveyor belt the injured felt pain in the area of his shoulder blade when he stood up. Diagnosed as thoracic muscle strain.

Employee tripped on a low mound of material on the floor and jammed his toe against the inside of his steel toe boot.

Employee was trying to put the South Bulk scope back together and material fell into his right eye. He had the bracket in his hand as he was looking at it, material fell from above, most likely from the scope. The scope was cleaned down before that job was started. There was not much of a breeze at the time of the incident, but it was in a west to east direction.

Employee was working near the back packhouse door unloading a truck. The door was open as product was being moved outside. The wind was blowing at a high rate and dirt/dust blew into employee's left eye.

Employee was cutting out jacket of Sealtite. Improper use of utility knife (cutting toward self, instead of away) caused injury when employee slipped with knife and lacerated left wrist. 5 stitches required.

Fall from ladders

Employee was using 3' ladder to access slides that switch from one silo to another. He was climbing ladder w/a pipe used to push the slide. Weight of ladder caused it to tip over backwards and employee fell to the ground, a distance of 3'. He landed on his left side, brusing his ribs.

Employee was on his knees cleaning clinker from under #31 belt, east side. When the employee went to stand back up, he twisted his left knee. Access to the east side of #31 belt is limited. Employee reported the incident at 14:30 pm and refused medical treatment at that time. He then requested medical treatment on Friday, March 6, 2009 at 7:00am.

1st Employee was walking out of the north doorway of the mill basement through the hanging curtains. Another employee operating the New Holland skid steer was driving into the basement at the same time. 1st employee was struck in the left leg by the bucket of the skid steer loader. Employee complained he had knee and back pain.

Employee was working to clean out hopper below #4 mill gypsum clinker feeders. He tripped over the cord on the floor, resulting in twisting his right ankle and knee also causing him to fall against a wall plate, injury his right shoulder

Employee was walking around NW corner of the storage building at the North end of the No. 3 packhouse. He slipped on a patch of ice and landed on his left wrist.

Employee was carrying a battery from the back of the truck to the track mobile. As he walked accross the railroad tracks, he felt pain on left side of his back.

Employee pushed up an empty wheel barrel. He took one step forward and caught his head on a piece of steel support for A belt line. His head was pushed backwards and down.He felt a crunching in the back of his neck. Incident occured on 12/18/2008, employee continued to do his job. However, as of February 10, 2009, he bacame restricted from his packing position.

Employee was working on #1 drag clinker silo when cover slid forward. Employee tried to catch cover and got finger caught under cover causing contusion to 5th finger of right hand.

EE was rotating a kiln using a ponymotor, 2 clutches are to be engaged. The 1st was engaged, EE was working at 2nd at the time of the accident. As the clutch was engaged, the pony began to labor and in order to avoid stalling, EE began to disengage the clutch. As he disengage it was backspinning driving the clutch plate in reverse, causing failure of the clutch.

Employee was putting freed clinker in silo through the reclaim system. Employee bent over to pick up a clinker lump and felt a pull in his back.

EE was changing a barrel of oil in the mill building, EE lost his grip on the barrel while trying to lift it. EE's fingers were caught between two barrels, about 1/8"" to 1/4"" space inbetween barrels. EE appears to have lost the top tips of his left index and middle fingers.

An employee went to throw a bucket of water at another employee. Employee raised their hand to block the bucket & the bucket struck her pinky finger on the left hand.

EE was inspecting block grease at 2nd pier of kiln #5. He slid the lid that covers the block grease area up near the kiln trunion tire. He rested his left hand in the lid area. The lid came in contact with the trunion tire, pushing the lid onto the finger of his left hand. Left pinky finger was amputated off at the end knuckle and ring finger also severed (but no bone loss).
