Adjusting and cleaning of a tail pulley on a 110' Masaba Conveyor LOTO was not performed by the EE before entering the tail pulley section of the conveyor. Belt was energized by the operator; EE was in tangled in the pulley from his feet to his mid section of his body. EE was fatally injured.

WALKING ON SOME ROCKS AND TWISTED KNEE. X-RAY WAS NORMAL AND EE RETURNED TO WORK MISSING ONE DAY.
