On 2/9/10 a water valve broke causing water to push employee in a low spot in the ground resulting in a knee strain. At the time he denied medical treatment. On 10/15/10 employee requested to seek medical treatment and was taken off work.
Employee was attempting to remove a plastic cable tie from inside a haul unit when his box cutter slipped resulting in a laceration to his right thigh. Employee received 7 stitches as a result of this laceration.

Employee, who is a truck driver, was walking near the scales toward the conveyor/ hopper when he tripped and fell to the ground. This caused him to hit his head resulting in a mild concussion and laceration to his head.

Employee was using a pneumatic impact wrench to tighten a bolt. He used his left shoulder to lean against the wrench for more leverage. This resulted in pain to his shoulder and chest. After seeing a medical professional this injury was not MSHA recordable; however the employee wished to get a second opinion at which time it became reportable.

Employee was using a prybar to install a roller on a dozer when he smashed his thumb between the roller and the equipment. At the time, the employee did not seek medical attention. It was not until 10/23/09 when swelling was apparent in his thumb that the employee wished to see a medical professional.

EE was operating a loader on 2/23/09 when she hit several potholes causing strain to her neck. On 2/27/09 while operating a loader, again she hit several potholes causing strain to her neck. EE did not report injury to supervisor until 3/4/09.
On 2/06/09 employee was removing bucket teeth with a hammer when he noticed a laceration on his left forearm. Employee used first aid to care for the cut. On 2/16/09 employee realized that laceration had become infected and sought medical treatment, at which point the injury became reportable to MSHA.
