Struck by flying object

Employee was wearing improper cut level of gloves when hammering metal on metal. Metal piece separated, cut through employee's glove and into the skin between two knuckles.

Dredge operator tripped on swing cable while stepping from dredge ladder to pontoon. Employee fell to the deck of the pontoon suffering a sprain/strain lower back.

Two miners were attempting to lift a conveyor belt scale that weighs approximately 60 pounds. One of the miners strained a muscle in lower back while lifting the scale.

Struck by falling object

Miner suffered a strain neck when ee was struck on the top of hard hat by a lump of dirt that fell from the top of an excavator bucket that was resting on the ground about to be greased. The injury was treated as first aid and after a week the injury did not get better. Miner sought medical treatment on 10/19/2016 and was placed on restricted duty starting 10/20/2016.

Employee stated he felt a pull and tightness in his left knee while climbing the stairs helping carry a metal strap. Did not want treatment until 20 days later, left knee sprain strain

Employee was removing a down spout from the chute under sand classifier when it slipped and pinched the tip of his left ring finger between the spout and the edge of the chute resulting in a laceration requiring stitches. Employee was wearing all PPE including gloves.

While helping guide a loader moving a 14 inch poly pipe the chain slipped over the loader bucket causing the pipe to fall and swept the EE feet and legs out from under him fracturing his left leg

EE reported he might have carpal tunnel has not been treated or reported. 9/17 EE went to hand specialist 10/01/12 receive doctor report with diagnosis treatment plan would be no loss time but EE was terminated before reported injury

The Red River had been flooded for 2 weeks leaving mud along bank edge EE stepped of loader and sunk knee deep in the mud injured left knee twisting to get unstuck.
