A miner was leaning out of the bucket to position a pipe when he decided he needed to be higher. He rang the bells to raise the bucket then failed to get the bells rang to stop the bucket. He was crushed between a guide rail timber and the bucket. The injuries included head lacerations, neck & spinal injuries.

While entering mine shaft, hit chain to lid with elbow causing lid to come down on hand.

While cleaning out bag house feeder motor. detached and in the shop area, rolled blade over top of finger crushing tip.

Employee was struck by a chunk of ore while climbing a ladder to exit mine. It cause him to lose balance and fall off ladder for about 3 feet. He fell with his back & side onto a landing board. He was able to catch his breath and climb out of mine. Was taken to hospital around 4:45.

Two miners were working ungerground on the mine slope. A miner at the upper portion of the slope attempted to move a 2x8x8 piece of lumber when it slipped from his hands, slid down the slope and struck the miner at the bottom of the slope above the right knee resulting in a bruise.

While using chipping hammer a piece of ore flipped up into his eye.

Coming down the stairs from pulverizer slipped and while grabbing the hand rail to catch himself from falling it pulled left shoulder.

While in dark mine picked up water bottle and took a drink but bottle was filled with methanol instead of water.

Manway timber fell out when employee stepped onto it from the ladder. He fell about eight feet.

Harrison Mine-fell down 50' slope and fell down and over four benches.

On way out of mine and ore fell on him.

While mining, ee placed a spool of wire on the timber above his head and it rolled off hitting him in the forehead.

EMPLOYEE WAS MOVING OLD AIR LIFT PIPE, WHEN HE STRAINED HIS LOWER BACK.

WHILE MINING, A PIECE OF ORE FELL DOWN THE DRIFT AND HIT HIM IN THE RIGHT SHOULDER.

WHILE MINING A ROCK CAME DOWN THE SLOPE, HITTING HIM IN THE CHEST.

WHILE RIDING IN THE SKIP GOING DOWN THE SHAFT, EE CAUGHT AN ARTICLE OF CLOTHING AND HAD HIS HAND PULLED IN BETWEEN THE BUCKET AND THE SHAFT. THIS RESULTED IN INJURING HIS RIGHT HAND.

WHILE PUTTING IN LAGGING, WAS HIT ON THE HAND BY AN AXE HEAD.

WHILE CARRYING BOARDS BACK TO THE DRIFT, EE STEPPED OFF A 6" STEP AND TWISTED HIS ANKLE.

WHILE PUSHING THE MONORAIL, IT CAME UP AND SMASHED HIS LEFT HAND.

WHILE MINING, PULLED MUSCLES IN ARM HAULING ROCKS.

WHILE WELDING ON THE SCREEN DECK UNIT AT THE SEPARATOR, STRAINED BACK.

WHILE MINING, TYLER FELL OFF A FLOOR AND HIT HISRIGHT SHOULDER.

WHILE MINING, A ROCK FELL AND HIT HIS RIGHT HAND.

WHILE HELPING TO MOVE A PRESSURE VESSEL, INJURED BACK.

SHOULDER JOINTS ARE WORN OUT.

WHILE PUTTING ROCK INTO THE BUCKET HE PINCHED THE RING FINGER OF HIS RIGHT HAND BETWEEN THE BUCKET AND THE ROCK BREAKING HIS FINGER.

LOADING ROCK INTO THE BUCKET HE INJURED HIS LEFT WRIST.

SLIPPED ON SLOPE INJURING HIS RIGHT KNEE.

HE BENT HIS THUMB BACK.

WHILE MINING HE INJURED HIS RIGHT FOOT: A ROCK FELL, HIT HIS HAND KNOCKING THE CHIPPING HAMMER OUT OF HIS HAND. THE MOIL HIS HIS FOOT CAUSING A LACERATION.

MOVING AN AIRLIFT PIPE HE PINCHED HIS RIGHT INDEX FINGER BETWEEN THE BUCKET & THE PIPE. HE HAS ABROKEN FINGER.

HAND GOT HIT WITH A CHIPPING HAMMER.
