Caught in, under or between a moving and a stationary object

A miner was leaning out of the bucket to position a pipe when he decided he needed to be higher. He rang the bells to raise the bucket then failed to get the bells rang to stop the bucket. He was crushed between a guide rail timber and the bucket. The injuries included head lacerations, neck & spinal injuries.

While entering mine shaft, hit chain to lid with elbow causing lid to come down on hand.

While cleaning out bag house feeder motor. detached and in the shop area, rolled blade over top of finger crushing tip.

Fall from ladders

Employee was struck by a chunk of ore while climbing a ladder to exit mine. It cause him to lose balance and fall off ladder for about 3 feet. He fell with his back & side onto a landing board. He was able to catch his breath and climb out of mine. Was taken to hospital around 4:45.
