Employee missed a step while climbing off the back of EE's mechanic's truck. EE fell to the ground, hitting EE's elbow. X-rays showed EE fractured EE's elbow.
Miner was using a grinder. A sliver of metal got behind EE's safety glasses and embedded in EE's right eye, requiring removal from EE's eye doctor. Miner was prescribed eye drops to use for one week and returned to full duty.
Employee was working on a mud pump, slipped in the mud and fell on left shoulder. Fracture to upper end of left humerus.
Air pin on trailer hitch was jammed, using hammer to tap pin. Pin released and left index finger was crushed between air pin and trailer frame. Open fracture to end of left index finger.
Miner was using a shovel to remove buildup of material when EE felt a strain in their back.
Employee entered feeder at wet plant to complete end of shift clean out, to manually unplug and remove materials build up. EE was shoveling in far corner of hopper when the material gave way. They lost footing and fell about one foot hurting their shoulder. EE stated that they regained themselves & started to climb out when a knee gave out and they fell again, about one foot.
Miner was assisting with changing a return roller when the roller slipped out of the bracket and pinched EE's right finger between the roller and the bracket, causing a laceration that required seven stitches.
Employee was assembling legs for conveyor belt on portable crusher. Leg fell out of EE's hands and struck EE's left foot and ankle. Contusion to left foot.
Machine had received a major repair, personnel was checking machine for leaks, or excessive vibration. While checking machine for a possible vibration issue I placed my hand on the top of a motor fan guard, the guard had a damaged area allowing my finger to come in contact with the motor fan.
Employee was doing patch welds on hopper, next morning (4/4/24) reported EE had received arc flash to both eyes.
Employee was cutting a rubber hose with a razor knife. Knife slipped cutting right upper leg just above knee.
Injured employee was assisting with maintenance of the wash plant. Other employee was backing up service truck and the bumper struck the injured employee in the right knee.
Miner was helping to change cutting edges on a loader. As cutting edge bolts were being removed, EE reflexively reached to catch a bolt that had spun free and received lacerations to hand from spinning bolt head.
Employee was using a pry bar to dig out stuck material on the belly belt, smashed right ring and pinky fingers between pry bar and stationary metal. Lacerations to both fingers, fractured distal phalanx on both fingers.
Miner was steam cleaning a piece of equipment when water/grease sprayed into right eye. Miner received medical examination to ensure no damage to eye after flushing.
Miner was working when EE became disoriented for several minutes. EE was taken to medical clinic and diagnosis of EE's condition is undetermined. EE's doctor has referred EE to a neurologist for further tests to determine underlying medical condition.
Miner was using a razor knife to cut belting material. Hand slipped and cut left index finger, requiring 11 stitches.
Changing a rear idler on the dozer, idler was stuck in place so employee tried striking it with a sledgehammer. Piece of metal flew off and embedded into the right side of employees chest. Foreign body removed and wound was sutured.
Employee was cutting through some electrical tape with a razor knife. Knife slipped and cut employees left middle finger. Deep laceration to left middle finger.
Employee was working on a 10' 11/2" steel pipe on a table with a vise. The pipe rolled and dropped on the edge of the table crushing employee's left pinky finger. Laceration to left 5th finger.
While driving a haul truck operator went over a rut in the road and jarred back.
Miner was helping to change out a screen on a shaker deck. The screen snagged EE's reflective vest, pulling EE off balance. EE caught self with EE's left hand and dislocated two fingers. Dislocation stretched the skin of palm and it tore, requiring stitches.
On June 21, 2022 employee was pushing in a screen rail and felt a pain in left hand, resulting in a strain.
Mechanic was arriving to work site on personal motorcycle. When EE entered site and turned toward parking area, EE ran over gravel that had spilled on the road and lost control of bike. EE crashed and broke ribs and right collar bone.
Miner was cleaning the floor with a water hose when they slipped and fell, striking their left elbow, resulting in a laceration requiring stitches.
Miner was grinding a metal guard so it could be welded back into place. The grinder hit a burr and bucked. Right forefinger struck grinding wheel, resulting in a laceration that required five stitches.
While changing a bearing, Miner hit bearing race with a hammer and a piece of metal broke off, cut through EE's glove and lacerated the middle finger of right hand, requiring stitches.
Employee was lifting oxygen tanks and strained lower back.
Employee was walking around skid steer and slipped on ice. Struck chin on bucket of skid steer, laceration to chin.
On Feb 1 employee was installing a roller on a dozer, on Feb 2 EE was changing a battery in a semi. At end of day noticed pain and discomfort in abdomen. Umbilical hernia. On 2/21/2022 determined this occurred on an MSHA site.
Miner was working on repairs when a piece of metal slipped from EE's hand and lacerated forefinger on right hand.
Employee was performing a pre-trip inspection on EE's trackhoe, as EE was stepping down EE missed the track and fell backwards off the equipment landing on a concrete pad. Struck EE's head and back. Non-displaced fracture at L2-L4 right transverse process. Employee was taken off work at this time, estimated four weeks.
Employee was removing a belly pan from a dozer. As employee removed the last bolt the pan fell on employee's right hand. Laceration and contusion to right hand required stitches.
Miner slipped on ice and fell, striking back on a rock, causing bruising and muscle strain.
Injured was thawing out a water valve. Steam built up in the valve and when it released the steam went under the safety glasses and irritated the injured's eyes, resulting in a burn to left cornea. Injured was treated by medical provider and returned to work.
Haul truck driver was dumping material at dump site. Truck went over the dump site and overturned at the lower level. Crews responded and notified Emergency Response. First aid was provided until EMTs arrived.
Was cutting lacing out on a belt when the knife slipped and cut hand.
Sampling sand from the drag pile, got stuck in the sand and was injured pulling self out.

employee was taking a cap off of a hydraulic line using a 24" crescent wrench. The wrench slipped off and hurt shoulder. Employee didn't seek treatment until 06/03/2019
Employee was walking on site, carrying a compressed gas cylinder of Oxygen. The ground was frozen. The employee stepped onto a soft spot of ground and fell forward. The gas tank struck the ground and the employee landed on top of the edge of the tank; striking the right side of their collar bone.
Slipped on ice while carrying objects and fell on shoulder.
A piece of bearing collar broke off after being hit by a hammer and punctured IW lower lip.2
IW was replacing bolts on a cyclone dewatering screen discharge lip. Employee was removing the bolts with a pry bar when the bar slipped. IW's hand slipped and hit the discharge lip cutting their thumb. They received 3 stitches.
Shoveling dry screen deck when a rock came off the feed belt and hit IW's finger breaking it.
Employee was walking between 2 cables at the conveyor line at the press. Left leg made contact with the cable fastener area & the frayed cable end ripped their pants & made a 3" cut on their leg. Employee received 8 stitches.
