Miner was using a shovel to remove buildup of material when EE felt a strain in their back.

Tripped over boards which had tipped over into walkway causing him to stumble and hit ribs on a metal table. Did not seek medical attention for several days. Condition did not improve.

Lab technician got out of his pickup to speak with operator at mine and slipped on the ice and strained right shoulder. Initially treated as first aid injury and technician returned to work without restrictions. Pain continued and work restrictions were put in place for technician restricting use of arm.

Clearing rocks from crusher frame under jaw, he lost footing and fell into rocks and shovel.

STEPPED OFF A CEMENT BLOCK ONTO A LARGE ELECTRICAL CORD AND TURNED ANKLE.

STEPPED ONTO TRACK OF DOZER TO CLEAN SILT OFF TRACK AND SLIPPED.

CLEANING OUT MATERIAL WEDGED IN V BELT ON MOTOR, MOTOR WAS SHUT DOWN, PULLED ON BELT FINGERS GOT BRIDGED BETWEEN PULLEY AND BELT.
