Miner was steam cleaning a piece of equipment when water/grease sprayed into right eye. Miner received medical examination to ensure no damage to eye after flushing.

Miner was using a razor knife to cut belting material. Hand slipped and cut left index finger, requiring 11 stitches.

Mechanic was arriving to work site on personal motorcycle. When EE entered site and turned toward parking area, EE ran over gravel that had spilled on the road and lost control of bike. EE crashed and broke ribs and right collar bone.

Miner was working on repairs when a piece of metal slipped from EE's hand and lacerated forefinger on right hand.

sampling sand from the cone about halfway up and sank into wet sand getting stuck. reported injury 6/13/20

Slipped on ice while carrying objects and fell on shoulder.

A piece of bearing collar broke off after being hit by a hammer and punctured IW lower lip.2

Shoveling dry screen deck when a rock came off the feed belt and hit IW's finger breaking it.

checking for holes in a cone liner, as it was spinning, a section of the liner cut IW face

Changing lowers on vsi, allen wrench slipped and the bolt head cut finger

Operating a different truck and felt that a different seat caused her to have back pain

Pounding wedges into place. Co worker missed wedge and sent a glancing blow on his lower back. Incorrect employee placement for job being performed.

Cutting a piece of belt for replacement and the knife slipped cutting his leg

Stepping from cab of dozer onto step. During work a rock had become stuck in the step which caused him to roll his ankle and foot.

Changing a cutting edge and bolt was stuck. He used his hand to hold bolt instead of a wrench and when it spun it cut through glove lacerating his hand.

running a dozer and while backing to make another push ran over a large rock jarring his back and neck

Working a pin into leg of crusher with hammer, pin flipped back and hit employee in eye.

Employee was walking and stepped into a hole and pulled his back.

a rock fell from a moving conveyor and struck him on the mouth chipping a tooth

sliding a screen across catwalk. the screen had a sharp piece of metal from vibration of shaker and he pushed it with his foot causing a very small puncture wound to his foot

Checking screens on a wet shaker and stepped over a wash bay onto a j bolt which penetrated into shoe and slightly into foot.

Changing a roller on a conveyor. EE was holding the roller as the bolts were removed. When the last bolt was removed the roller, and co worker working above partially fell onto his shoulder and arm.

Stepping off of last step from Cat 380H loader and rolled ankle on a rock.

EE was replacing a screen cleaning bar when a piece of cutting edge bounced onto his finger smashing and cutting it.

Holding a bolt with his fingers instead of a wrench and tried to spin it free with an air wrench cutting hand.

Employee was operating a loader that ran over a rock, he struck his head on the column in the loader.

Employee was pressing out a cone liner. Flanges broke & metal I-Beam flipped up & hit him in the jaw.

Lifting a tail pulley with another employee - strained hip and leg.

Changing a ripper tooth on a dozer, employee was pounding the tooth into place and a small piece broke off and hit him in the chest impaling him about 1/2 inch.

Shoveling dirt piles he felt shoulder pain.

Replacing a belt, cutting along a template. Had to nail template to belt, missed and hit finger.

Lifting electrical wires to hang on the side of a conveyor. Strained back while lifting.

Pulling cart with oxy-acetylene rig on it, felt a muscle pull.

Climbing off of loader, turned on last step and foot got caught twisting and breaking it.

WAS TYING AN AIRLINE BACK WITH TIE WIRE, LINE SLIPPED AND WIRE POKED HIM IN THE EYE. HAD REMOVED SAFETY GLASSES BECAUSE THEY WERE DUSTY.

HOLDING A PIECE OF STEEL TO PROTECT ADJACENT HYDRAULIC LINES FROM DRILL BIT, DRILL BOUND UP AND THE CORD WITH THE KEY CHUCK HIT HIM IN THE MOUTH CHIPPED A TOOTH.

TAKING SHIV OFF OF CRUSHING CONE, CRANE LIFTED MOTOR AND SHIV FELL ONTO EMPLOYEE.

POSSIBLE CARPAL TUNNEL PRO LEM--USE OF IMPACT TOOLS, HAND TOOLS.

STEPPED OFF LOADER STEP AND ONTO A ROCK, ROLLED ANKLE.

CLEANING UP USING A BOBCAT F/E LOADER, PIECE OF METAL FOLDED AS IT WAS BEING SCOOPED AND HIT OPERATOR ON THE KNEE.

SHOVELING FELT PAIN IN WRIST

DRIVING PIT TRUCK WENT TO PASS LOADER THAT WAS PARKED AND HIT IT

AIR ARC WELDING--EXCESSIVE SMOKE COMING OFF SITE--FLASH BURNS TO EYES.

WIND BLEW A PARTICLE INTO EYE.

SLIPPED OFF LOADER AND ONTO A ROCK. TWISTED KNEE

PUTTING V-BELT ON THE FEEDER MOTOR, MOTOR BEING SUPPORTED BY BAR WHICH SLIPPED AND INPACTED FINGER.
