Employee was sitting in the breakroom and stood up. When he started walking he felt pain in the upper part of his right leg. As he walked the pain worsened. He was taken to the emergency room and diagnosed with a strain/sprain to the muscle in his leg.

Showing 50 of 56Employee was installing bucket elevator's equipment when the bar he was holding slipped causing the bucket to jerk his arm & shoulder. *Note: Employee was released to return to work with restr ictions. He never returned to work and therefore was terminated.
Utility operator was preparing to wash out the rail scale. The loader operator was weighing the loader on the scale. The loader operator announced on the radio that he was moving the loader off the scale. He slowly moved forward when he observed the utility operator on the ground. The utility operator apparently fell and was crushed by the front left tire of the loader.
Dust from grinding got under contact lens. Employee was wearing safety glasses. Notified 4/21/2006
Employee was tightening a section of 3" conduit and felt a "pop" in his left hand, followed by a burning sensation. When he awoke the next morning his hand was swollen. He then reported the incident and sought treatment at the local emergency room.
Employee was using his finger to remove a nut that was stuck in the socket of an impact wrench when he accidently turned the impact wrench on. This drove the nut onto his left index finger.
Putting up light fixture on outside of building and fell to ground.
Employee was hooking up a trailer to the ball hitch on the bumper of a pickup truck. He was moving the tonque of the trailer back and forth to get the tonque to seat when it mashed his finger.
Sprained his neck while working on a fuel filter.
Employee was removing material with a hand shovel when he grazed a sharp edge of metal with his arm.
EE was repairing pipe threader attempting to prime oil pump when finger came in contact with exposed gears.
Miner was climbing railcar, rung broke causing miner to fall.
EE welding on top of storage tank when ignition occurred burning his left arm. storage tank integrity may be in question.
Fumes in 20,000 gal. #4 oil storage tank ignited when a welding spark reached the fumes. Resulting flash explosion knocked employee from on top of tank.
While working in low headroom area employee hit his head on a steel beam.
EE dumped at the crusher and did not lower the bed on the haul truck. The bed impacted the brow at the second level entrance. Jamming the cab of the truck into the back.
EE was tightening clamp on waterhose with a screwdriver. Screwdriver slipped jabbing into left hand palm.
DESCENDING STEPS IN AMICAL MILL ROOM, FOOT SLIPPED, TEAM MEMBER FELL AND CAUGHT LEFT HAND ON METAL ENCLOSURE, CUTTING GASH IN LEFT PALM.
Repetitive action
Employee notice that his eyes were irrated after being around diesel smoke. He was take to the doctor and given prescription eyedrops. The doctor was unsure of the cause of the irritation.
Employee was shoveling under the crusher when he felt a sharp pain in his lower back. The doctor said he had a back strain.
Team member was cutting approx. 8 inches off a 4 foot piece of 8 inch pipe. the balance of the 4 foot piece fell from the pipe stand and landed on the team member's right foot.
Changing a blow line from 1 pipe to another. There was no air pressure on the line when the accident occurred. The line slipped off the pipe, whipped around and hit EE in the face. EE has a deviated septum; will have open nasal reduction surgery 08/10/04.
EMPLOYEE SLIPPED AND FELL ABOUT 2-3 FEET AND LANDED ON HIS HAND. THIS CAUSED HIM TO SPRAIN HIS RIGHT HAND. HE WAS WALKING ON A CONCRETE PAD OF A SILO AND SLIPPED OFF THE PAD ABOUT 2-3 FEET.

Ee and other maintenance members were repairing the ramp from the hatch opening station. The walkway grating was being lowered into the walkway framework when it fell onto the team member's left hand little finger. The damage was only to little finger.
FOREMAN ATTEMPTED TO WALK ON THE EDGE OF A MUCK PILE TO STAY OUT OF A WATERHOLE. FOREMAN SLIPPED ON A ROCK AND FELL ONTO A ROCK CUTTING HIS WRIST.
EE WAS CUTTING SOME SKURTBOARD FOR THE SCREENS AT THE AGG PLANT. HIS KNIFE SLIPPED FROM HIS HAND AND CUT A WOUND ON THE INSIDE OF HIS LEFT FOREARM. THIS REQUIRED FOUR SUTURES TO CLOSE THE WOUND. THE DOCTOR RELEASED HIM TO WORK THAT SAME NIGHT.
THE INNER CAGE MILL HAMMERS AND CAGE WERE BEING ASSEMBLED. SMALL WOODEN BLOCKS WERE BEING PLACED UNDER EACH BOLT TO SUPPORT IT IN THE RECESSED POSITION. AS THE BAR WAS BEING USED TO RAISE THE RING, PRY BAR SLIPPED AND THE RING DROPPED APPROX 2 INCHES MASHING THE TIP OF THE 4TH FINGER. FINGER TIP BONE WAS FRACTURED.
EE WAS SHOVELING A BUILD UP OF MATERIAL AND WAS PULLING THE SHOVEL TOWARD HIM WHEN HE INJURED HIS LEFT GROIN.
EMPLOYEE WAS LYING ON HIS LEFT SIDE USING A CUTTING TORCH TO BURN A HOLE IN THE GROUND. AS HE WAS BLOWING THE HOLE, A PIECE OF HOT SLAG GOT IN HIS EAR.
A LOAD BEING LIFTED WITH NYLON KEVLAR FIBER FLAT SLING OVER THE EDGE OF THE FORK TIRES. AS THE TINES WERE SLIGHTLY POINTED UPWARD WHILE LIFTING THE LOAD, THE WEIGHT OF THE SAID LOAD CAUSED SLI NG TO SLIDE DOWN TINES & CUT AND BURNED THE FIBER, CUTTING THE SLING INTO. THE LOAD THEN FELL ONTO THE RT LEG OF THE INJURED EMPLOYEE.
WHILE CLEANING ON TOP OF BALL MILL SEPARATOR ROOF I FELT MY BACK PULL. I WAS USING A PICK AND SHOVEL.
EE WAS WORKING ON REPLACING THE WEAR LINING IN THE CRUSHER FEEDER. HE BECAME OVERHEATED.
EE WAS USING THE BOBCAT TO REPLACE SECO FEEDER BELT. IT WAS RAINING AND UPON EXITING THE BOBCAT HE STEPPED ONTO THE BUCKET AND HIS FOOT SLIPPED CAUSING HIM TO FALL ON HIS LEFT SIDE INJURING HI S KNEE, HIP AND SHOULDER.
REPLACING JACK SHAFT AT ROLLER MILL, SHAFT STRUCK EE ON THE LEG.
A SMALL PART OF A ROCK CUT EE'S HAND REQUIRING TEN STITCHES.
INSTALLING A REDUCER TO RADIATOR USING A PIPE WRENCH. WAS TIGHTENING REDUCER TO RADIATOR, PIPE WRENCH SLIPPED & CUT THUMB ON LEFT HAND.
THE DRIVER SLIPPED FROM THE TOP OF THE TRAILER WHILE CLOSING ONE OF THE DOME LIDS, SLIDING UNDER THE SAFETY RAIL AND CATCHING ONTO A SAFETY RAIL ON THE TRAILER. AFTER HOLDING ON FOR A TIME HE LOST HIS GRIP AND FELL ABOUT SEVEN FEET TO THE PAVEMENT AND LAY THERE FOR ABOUT ONE AND ONE-HALF HOURS UNTIL HE WAS FOUND.
EE FELL OFF LADDER & CAME TO REST OVER DOOR OF BOTTOM DUMP RAIL CAR. EE WAS INSIDE CAR AT TIME OF INCIDENT, INSPECTING SAME. WHILE IN THIS POSITION, HIS FOOT WAS IN THE AREA OF THE DOOR, WHICH WAS CONCURRENTLY BEING CLOSED FROM OUTSIDE OF COMPARTMENT. UPON BEING ALERTED, OTHER EMPLOYEES OPENED DOOR & HE EXITED RAIL CAR.
EE WAS STANDING IN THE PLANT SHOP TALKING TO PLANT PERSONNEL WHEN HE FAINTED. HE FELL TO THE FLOOR INFLICTING A CUT ON HIS FOREHEAD. SUTURES WERE REQUIRED.
HIS FAILURE TO PROPERLY POSITION HIE FEET WHILE TURNING THE DRUM CAUSED HIS FOOT TO SLIDE BETWEEN THE DRUM AND THE CRUSHER WALL.
TEAM MEMBERS WERE TO HAUL MATERIAL FROM SI6 TO DUMP AREA. THE TAIL GATE FAILED TO LATCH SO TEAM MEMBER TOOK THE TAILGATE AND PREPARED TO SLAM IT SHUT. HIS FINGER WAS CAUGHT IN THE LATCH ASSEMB LY.
PULLED BACK MUSCLE WHILE CLIMBING A LADDER.
EE COLLAPSED WHILE STANDING IN THE MILL CONTROL ROOM AS HIS MORNING SHIFT WAS BEGINNING. HE WAS CONVERSING AND GAVE NO INDICATION OF DISTRESS WHEN HE COLLAPSED. ATTEMPTS TO REVIVE WERE UNSUCCE SSFUL AND HE WAS PRONOUNCED DEAD AT THE HAMILTON COUNTY EMERGENCY ROOM. CAUSE OF DEATH WAS A BLOOD CLOT IN HIS LUNGS.
EE WAS OPERATING A PNEUMATIC HAMMER ON CEMENT. FLYING DEBRIS WAS ABLE TO ENTER RT EYE DESPITE EE HAVING ON SAFETY GLASSES
EMPLOYEE WAS MOVING A 55 GALLON DRUM OF OIL FROMA PALLET ON A FORKLIFT INTO THE OIL SHED. DRUM TIPPED SLIGHTLY AND EMPLOYEE REACTED TO PREVENT DRUM FROM TIPPING OVER. THIS ACTION CAUSED HIM TO SCRAPE HIS RIGHT ARM AGAINST THE DOOR FRAME, RESULTING IN A LACERATION ABOUT 2.5" LONG.
EMPLOYEE WAS DESCENDING A VERTICAL LADDER BETWEEN CONTROL ROOM LEVELS; HIS FOOT SLIPPED FROM THE3RD RUNG FROM THE BOTTOM CAUSING HIM TO LAND WITH FORCE ON HIS RIGHT LEG.
ROCK FALL FROM ROOF COLLAPSED CAB OF TAMROCK MINIMATIC DRILL KNOCKED EE TO THE FLOOR.
EE WAS CLEARING THE WALKWAYS AROUND THE CRUSHER PRIOR TO STARTING CRUSHER. HE WAS TOSSING LARGE PIECES OF ROCK OFF THE WALKWAY AND CAUGHT THUMB BETWEEN THE ROCK AND THE HAND RAIL.
