While installing a driveshaft in haul truck it slipped from EE's hand and struck EE in the shoulder.

Reportable hearing loss in right ear.

While loosening a nut with a ratchet and socket, socket slipped off nut pinching right pinky finger between ratchet handle and the ground, lacerating finger.

EE was prying a conveyor head pulley in preparation for assembly, the pry bar slipped and EE fell into conveyor frame, pinching and lacerating the tip of their right index finger.

While working on the bench, was taping holes upon pulling the product hose, felt pain in left wrist (1st day of treatment 7/29/22)

While cutting steel with acetylene torches EE's shirt caught on fire. EE suffered burns/blisters on hand and stomach.

While assisting another EE in lifting a banding cart, EE felt pain in their back.

Removing rocks from plugged head pulley discharge box, EE initially reported right shoulder strain (9/10), worked 9/7, 9/10, & 9/11. On 9/11, EE sought medical treatment for left shoulder pain.

While cleaning syntron pan a bar fell and struck EE on the chin causing a contusion to chin and lip.

While removing rock from discharge conveyor EE slipped and struck head on sledge hammer

While moving full barrels of oil, EE managed to pinch wrist/forearm between barrels. Pinch/crush injury, no fractures.

EE ran truck into berm

EE complained about a sore right knee from driving a loader.

EE complained about a sore right elbow 2 days later after unplugging a crusher feeder.

While nailing a splice template to a conveyor belt EE missed nail and struck his left thumb.

While lifting heavy piece of steel EE felt pain in neck and right shoulder.

While connecting hoses to Sanvic crusher, EE stood up and cut head on a piece of channel iron.

While sorting large rock EE felt dizzy and shortness of breath. EMS was called for potential heart attack (EE has history). EE was released to full duty the following day.

While repairing a spring on a truck a foreign object fell in EE's eye. He was wearing safety glasses. Went to ER for eye wash and exam. Returned immediately to work.

While disassembling a crusher a wrench on a bolt slipped off the bolt striking employee above left knee. Employee did not report that he was injured until 7/13/2015

While pounding wedges into crusher to disassemble it, a piece of the steel wedge broke off penetrating the employee's abdomen. Employee has a follow up outpatient surgery to remove the object.

Haul truck driver exited CAT 773 Haul Truck to relief himself while waiting to unload haul truck into primary crusher. Employee returned to the haul truck and while climbing back up the haul truck, he lost his footing and fell off ladder approximately half way up the ladder. Employee sprained right wrist in fall to the ground.

Working to change the seal on an oil leak on the screen deck. Cut a hole with torch on Seco deck. Used a come-a-long to take the counter balance off. A spark must have gone into bottom of screen deck & smoldered. Flared up at 9:30.

While reaching to remove a rock from conveyor EE felt a sharp pain in his back, 5-10lb rock.

While cleaning under a conveyor EE pinched finger between conveyor and walk behind bobcat.

Profiling a face when rocks let loose from the face and fell, turned to run, fell scraping knees and jammed wrist.

EE was sorting stone in the pit with a loader, EE stopped and exited the loader, when stepping off loader EE stepped on a rock and rolled foot over causing a broken bone on the top of the arch of right foot.

After exiting his pickup truck ee slipped on ice and twisted his left knee.

EE was in caterpillar skid steer, another ee went up to side of skid steer to inform EE that he needed some belting, EE did not realize his foot was between tires on skid steer when EE drove off skid steer tire drove over EE right foot.

While standing and talking with another employee, EE fainted

While removing rocks from jaw crusher by hand EE felt pain in left wrist.

While ee was installing a conveyor belt, EE was using a cutting torch to remove bolts, the torch somehow got entangled in sleeve of leather welding jacket causing torch head to pull into his sleeve causing second degree burn to his left forearm.

Vendor ee was moving tires from a loader. He was using an excavator to remove tires. EE & another ee (plant mgr) had discussed need to put blocks under loader to crib it up. Loader moved & counter weight hit him in leg/side. 911 called

EE jammed finger into heater/fan to see if it was working, it was and fan chopped off the tip of EE's finger.

While cutting conveyor belting with a utility knife the knife slipped cutting ee left thumb, required 2 stitches.

Driller, was doing maintenance on his drill, slipped off a step and hit the handrail with his left side (ribs). I had an employee drive him to occupational health facility. X-rays were negative, just bruising. Tried to work through it, first lost day to workers comp 8/28/10, today. He has a Dr.'s appt 9/2/10. Restricted duty, lost work days.

Driver has complained about air conditioning in his truck not being adequate. Necessary repairs have been made to unit.

While grinding metal a foreign object got around safety glasses and into EE's eye.

After checking the engine oil in the machine, EE walked along the tracks to enter the cab, EE then reached for the door, slipped off the tracks and fell to the ground causing multiple fractures to EE's right heel.

While hanging conduit for new conveyor EE hit elbow on one of the conduit hangers severly bruising elbow.
