Fall from machine

While removing radiator from Cat 988B loader, EE inadvertently stepped off the counter weight and fell to the ground approximately 4.5 feet fracturing left elbow and possibly left hip.

While replacing a screen on wash plant another EE was using a pry bar to align the new screen, the other EE did not see EE's hand in the way and struck EE's hand with the pry bar causing a laceration between EE's ring and pinky finger of EE's right hand. 2 stitches at ER.

Struck by falling object

While pulling pin from bucket, pin fell on hand breaking the left little finger and lacerating same finger - broken finger with 3 stitches.

While cutting steel with acetylene torches EE's shirt caught on fire. EE suffered burns/blisters on hand and stomach.

Removing rocks from plugged head pulley discharge box, EE initially reported right shoulder strain (9/10), worked 9/7, 9/10, & 9/11. On 9/11, EE sought medical treatment for left shoulder pain.

While cleaning syntron pan a bar fell and struck EE on the chin causing a contusion to chin and lip.

While removing rock from discharge conveyor EE slipped and struck head on sledge hammer

While moving full barrels of oil, EE managed to pinch wrist/forearm between barrels. Pinch/crush injury, no fractures.

While disassembling a crusher a wrench on a bolt slipped off the bolt striking employee above left knee. Employee did not report that he was injured until 7/13/2015

Struck by flying object

While pounding wedges into crusher to disassemble it, a piece of the steel wedge broke off penetrating the employee's abdomen. Employee has a follow up outpatient surgery to remove the object.

While ee was installing a conveyor belt, EE was using a cutting torch to remove bolts, the torch somehow got entangled in sleeve of leather welding jacket causing torch head to pull into his sleeve causing second degree burn to his left forearm.

WHILE ATTEMPTING TO REMOVE A CONVEYOR, THE MECHANISM THAT WAS STABILIZING IT FAILED. AS A RESULT, THE CONVEYOR AND THE INDIVIDUAL DROPPED CAUSING THE EMPLOYEE TO LOSE HIS FOOTING AND BALANCE RESULTING IN HIM FALLING ON HIS RIGHT ARM ONTO THE CONVEYOR. THIS RESULTED IN THE EMPLOYEE BREAKING HIS UPPER RIGHT ARM.
