EE was prying a conveyor head pulley in preparation for assembly, the pry bar slipped and EE fell into conveyor frame, pinching and lacerating the tip of their right index finger.

EE was in caterpillar skid steer, another ee went up to side of skid steer to inform EE that he needed some belting, EE did not realize his foot was between tires on skid steer when EE drove off skid steer tire drove over EE right foot.

AS THE EMPLOYEE WAS DRIVING IN THE QUARRY PIT HE BACKED UP TO STRIPPING PILE. THE BOX WAS UPRAISED AND WHEN HE PULLED FOREWARD WITH BOX RAISED THE TRUCK TIPPED ON ITS SIDE. THIS CAUSED THE EMPLOYEE'S LEG TO STRIKE THE SHIFTING LEVER IN THE CAB.

EMPLOYEE WAS PULLING A TUBE OFF OF THE LIME TRUCK AND AT THE END OF TUBE WAS A HOT STEEL WIRE THAT HIT HIS RIGHT HAND AND BURN A QUARTER SIZE PIECE OF FLESH.

EE WAS REMOVING A 50TON JACK FROM BENEATH A 980 LOADER AFTER INSTALLING A TIRE. WHEN REMOVING THE JACK, THE BLOCK SLIPPED & THE JACK FELL DOWNCRUSHING HIS LEFT PINKY BETWEEN THE BLOCK & JACK
