While employee was cutting the rubber insulation from around the wires in the motor junction box, their retractable utility knife cut through abrasion-resistant glove on left hand, resulting in a laceration of thumb. They immediately went back to the Electrical Shop to clean and perform basic first aid on the wound.

EE was walking in the crosswalk from the Employee Parking Lot towards the Main Office. When they got to the office sidewalk, they placed their left foot up on the sidewalk and was beginning to take a step up with their right foot when they lost balance and fell backwards striking their head on the crosswalk pavement.

EE was performing inspections when they noticed dust burping from the vent line above the feed bin on the fourth floor of the Preheat Tower. They said they looked up to identify where it was coming from when dust vented out and landed just above their glasses under the brim of their hat. Dust entered their left eye causing irritation. They received antibiotic ointment for the eye.

While waiting on water removal process to be completed, EE picked up an air hose and began walking backwards to remove slack from the hose and stepped into the manhole causing employee to partially fall into opening striking lower back

The employee's tip of pinky finger on left hand became pinched between the bottom of the cover and a metal flange as they placed the cover on the ground. The employee's fingernail was almost avulsed from the nail bed

On February 4, 2026, an ACME field employee reported to EE's foreman that EE's right forearm felt strained but did not believe medical attention was needed. EE later requested medical attention, stating EE's forearm began hurting after using an air hammer for two hours removing excess concrete.

EE was walking towards EE's office when EE tripped on a rug causing EE to fall forward landing on both knees. EE then noticed blood seeping through EE's jeans at the right knee. Upon inspecting knee, EE noticed a laceration measuring about 4 inches in length across EE's right knee. EE went to Mercy Jefferson ER where EE received 12 stitches to close the wound.

Employee was working near the doorway at the alkali spray tower dump box when wind blew dust under EE's safety glasses. This caused EE's left eye to become irritated. This did not become reportable until the employee went to the doctor on 1/9/2026, when the doctor prescribed eyedrops.

EE was washing out the raw mill posimetric chute with a water lance. The water hose became entangled with the door, causing the door to begin closing. The door struck the employee's right right wrist resulting in a contusion. The injury became reportable on 11/18/25 when the doctor performed professional medical treatment during a follow up visit.

EE was inside manlift basket using a steel lance attached to a vacuum hose to clear material from apron feeder below. While vacuuming the material the hose became stuck and EE tried to free it by yanking on the lance. The hose broke free and EE's hand flew back striking the corner of the basket's railing breaking the 5th metacarpal bone in EE's right hand.

Employee was bending over to pick up a trash bag from a box that was on the floor. The employee did not notice a metal pipe that was next to the box on the floor and as the employee bent down to pick up a trash bag, EE's left forehead came in contact with the end of the metal pipe resulting in a laceration to the top left side of EE's forehead.

EE was pulling a Wright dump hopper from under the Clinker Cooler. While navigating through the "I" beams the hopper struck the edge of a beam causing the hopper to stop suddenly. EE's momentum continued backwards while gripping the hopper during which they heard a pop in left shoulder. EE immediately felt sharp pain in their shoulder/neck and reported it to a Supervisor.

Employee was sitting on an I-Beam that was stacked on top of another I-Beam with wooden cribbing between the two I-Beams. Their left hand was down near the space between the 2 I-Beams. The wooden cribbing broke, pinching the tip of their left thumb.

As the employee was stepping out of the raw mill, the employee placed a hand on the raw mill tire hook and disrupted the accumulated dust causing it to become airborne. The airborne dust entered the airstream of the cooling vent fans and blew onto the employees face and under safety glasses causing dust to enter left eye.

While cleaning in the 010 Conveyor area, employees noticed unusual material on the floor, but did not think it was concerning, assuming it was safe since spills or hot materials are normally barricaded. As one employee stepped to retrieve a shovel, the material gave way, and hot material entered their boots, causing a second-degree burn.

Supervisor noticed smoke in the alternative fuel storage pile, instructed a loader operator to move some material to get to the smoking pile of material. The material was subjected to the atmospheric conditions, it was oxygenated, igniting the TDF. Local fire department was contacted and arrived on scene and extinguished the fire.

When they removed the bracket that was over their head they felt something pull in their left shoulder and arm gave out. They have pain in their left shoulder. They saw doctor on 8/5/25 and started restricted duty on 9/8/25.

EE helped two workers lift a 4x8 screw lid back on top of its housing at Silo 17 Screw Floor. EE and workers lifted the lid onto the housing and began to push it into place when they heard a pop and felt slight pain in left shoulder. EE asked to see a doctor after pain persisted for 10 days. EE had surgery on 09/02/25 to correct a torn rotator cuff and bicep tendon.

EE was moving the 4 separator elevator gearbox with the help of another millwright on a wagon. The Millwright in the pulling positions twisted their knee and felt a sharp pain that travelled through knee and leg.

EE used a hammer to strike the head of a 10" chisel to free a stuck idler on a belt conveyor. When EE struck the chisel EE felt a sharp pain in EE's left forearm and EE's arm bled. EE was taken to the hospital where EE received treatment for a foreign body puncture (metal from the chisel head). EE received two stitches after the doctor tried to unsuccessfully to remove the metal.

During the heavy rain down fall near the end of the shift on 2nd shift, a rock from the rock pile came sliding down the pile and struck the right-side window of loader, breaking out the side window and striking operator on left knee area causing a laceration to knee.

EE was attempting to unplug a triple gate at the coal mill with no success. EE then introduced EE's hand into the triple gate. The gate activated, nipping the tip of EE's right middle finger.

Employee was working on housekeeping around the plant. While working with the vac-truck operator employee was giving hand signals to the driver during the backing up process. While doing this their feet slipped and they began to fall. during this process their right leg became entangled underneath them and they broke their leg.

On 5/13/25 EE lifted an oil bucket onto a truck with right arm and felt pain in shoulder. EE told their supervisor but didn't seek medical evaluation. When the pain persisted EE requested a doctor's appointment and was seen on 06/25/25 where they were treated for strain of rotator cuff & prescribed PT. EE had MRI on 8/1 and then surgical repair of right rotator cuff on 11/20/25.

At this time, EE's foot went into a depression in the walkway, causing EE's knee to twist awkwardly. EE felt something pop in EE's knee and immediately began to feel pain
