The employee said he had a rock stuck in his tire. He said rock was size of a football. He kicked the rock, breaking it. A piece of rock fell and hit him on the foot breaking his steel toe in half. I asked to see his boots that the steel toe broke and he stated he threw them away and the trash was already picked up so he couldn't bring them in.

Showing all 9Employee was working in the dump hopper tightening the bolts in the grizzly bars on the feeder. A rock fell off the hopper ledge approx. 36" and smashed his hand against the feeder. The doctor said his hand is swollen and he broke his finger.
Reset breaker in box, breaker blew, burnt left arm.
Employee was helping change a skid steer tire when a pry bar slipped striking him above the right eyebrow. It required six stitches at the local ER. He returned to work at 11:30 am.

THE MECHANIC OPENED THE HOOD TO CHECK OUT A MOTOR PROBLEM WHEN HE SHUT THE HOOD WITH ONE HAND HIS OTHER HAND WAS RESTING ON THE MOTOR WHERE IT WAS CAUGHT BETWEEN THE HOOD AND MOTOR.
R-50 HAUL TRUCK ROLLED BACKWARDS DOWN RAMP FROM QUARRY HOLE WENT OVER BERM AND FELL APPROX 25' OVER LOWER HIGHWALL LANDING ON ITS SIDE. ACCIDENT WAS FATAL.
