R-50 HAUL TRUCK ROLLED BACKWARDS DOWN RAMP FROM QUARRY HOLE WENT OVER BERM AND FELL APPROX 25' OVER LOWER HIGHWALL LANDING ON ITS SIDE. ACCIDENT WAS FATAL.
