R-50 HAUL TRUCK ROLLED BACKWARDS DOWN RAMP FROM QUARRY HOLE WENT OVER BERM AND FELL APPROX 25' OVER LOWER HIGHWALL LANDING ON ITS SIDE. ACCIDENT WAS FATAL.

Employee was working in the dump hopper tightening the bolts in the grizzly bars on the feeder. A rock fell off the hopper ledge approx. 36" and smashed his hand against the feeder. The doctor said his hand is swollen and he broke his finger.

THE MECHANIC OPENED THE HOOD TO CHECK OUT A MOTOR PROBLEM WHEN HE SHUT THE HOOD WITH ONE HAND HIS OTHER HAND WAS RESTING ON THE MOTOR WHERE IT WAS CAUGHT BETWEEN THE HOOD AND MOTOR.
