Employee was installing a water bag in a cut to open a new slab. While pushing on the water bag, EE's hand slipped on the side of the bag due to wet condition. Employee's cut is on the palm on the left-hand side. Sent to the ER and got 14 stitches and 10 days rest.

MINER FAILED TO ENSURE THE GRANITE BLOCK WAS BLOCKED AGAINST MOTION WHILE PERFOMING SECONDARY BREAKAGE.
The wire saw operator was in the process of installing a metal wedge under a granite block. While hitting the wedge with a sledge hammer, a piece of the block (85lbs) came loose and partially hit the miner's left shoulder.
EE was in the process of opening a 6' thick slab. Water bags were installed at the top of the slab. EE was not wearing a safety harness and was about 20' high. Slab broke and fell down & hit EE and crushed him.
Was chaining a block when a piece fell off of the block. Crushed right thumb and lost fingernail.
Fall from excavator catwalk while trying to remove a tarp. Land on right shoulder resulting pain. Sent to the ER to get X-Ray done. No fracture was found.
Employee was cutting metal with a grinder and disc shattered and penetrated right arm.
EE was pounding on 18" wedges to split a stone. EE missed and the hammer bounced back and split head open. EE cut forehead.
EE was running the quarry saw when a wire broke. EE was pulling the wire and walking backwards when EE fell on some grout. This was human error, EE has done this job for months. EE fractured ankle.
Went to step up onto crane, felt something pop in left foot.
Was pulling on a rail when ee heard a pop, cannot lift shoulder.
Jumped from one ground level to another three feet lower, rock on higher level broke loose and fell after employee jumped landing on lower leg.
EE was descending a ladder when EE slipped on the 3rd or 4th rung from the bottom; extension ladder shifted with EE's momentum and slipped off the square block it was resting against. Wet & possibly clogged boot tread contributed to the slip on the ladder rung.
Miner got dirt particle in eye while cleaning heating unit

Miner's left eye became red and blurry. Diagnosed with conjunctivitis.
Two miners were trying to get a cutting cable unstuck from a cut. 1 man on top of the bench and 1 at the bottom. The cable got hung on the high wall and the man at the bottom pull on the cable to get it free. When it did a rock 2" of diameter fell from the high wall and hit the miner who was looking up at the same time.

Air hose disengaged from drill unit cutting miner on thigh and pinning him under control panel.
Air line hose connection released from jackhammer striking miner on bridge of nose and teeth 3 times.

Miner tripped over himself hurrying to attend wire saw. While extending arms to break his fall, he broke bones in right hand and wrist.
He was pushing curb down the line on rollers all day. When he got home, he showered and then began feeling pain in his lower back.
Moving a 20 lb rock by hand - smashed right index finger tip
Employee lacerated right finger on corner of water bag. Resulted in sutures in finger.
He was at the middle level of the quarry pulling chain up from the tension car at the bottom of the quarry (chain was approx. 100'L) when he felt a pull in his lower back.
He had his left foot on a post about a foot off the ground and was pulling in diamond wire off the wire saw to check the crimps when he felt a tug in his groin area on the left.
MINER STRUCK A STEEL WEDGE WITH A SLEDGEHAMMER SHATTERING THE WEDGE SENDING STEEL FRAGMENTS INTO MINER'S NECK.
He was in a work box using a jackhammer in the quarry. The box began to shift outwards from the wall so he let go of the jackhammer and grabbed the railing. The box shifted back towards the wall pinching his right middle & index fingers between the railing and the wall.
Setting up a stone to saw. Pushed it in & hit clamp instead of stops. Tried to free it causing the clamp to knock stone forward & his fingers on left hand were caught between the stone & rollers.
He stepped out of the company van and didn't realize the ground outside the vehicle was uneven so when his left foot hit the ground he rolled his ankle.
He was driving down into the quarry when he stopped to speak to another employee. He got out of his truck and walked 4-5 steps when he slipped on snow covered ice and fell on his back. The back of his head hit the ground. He was briefly dizzy and disoriented and at first had no feeling in his hands which quickly subsided.
He was using a jackhammer all day during 9 hr. shift. Next morn. he woke to pain in lower back. Pain increased over the week so he reported it. Doctor diagnosed lower back strain May 5 & put on modified duty. Currently still on modified duty until May 28-no lifting over 40 lb.
Employee was trying to bend a piece of round steel when it rolled off of the work table and landed on his left foot. Employee was treated by a doctor who diagnosed a contusion. He was placed on modified duty until seen by an orthopedic doctor. The orthopedic doctor placed him on modified duty "until patient feels able"
The driller was shaking the drill to get it to move and felt a pain in his left shoulder.
PIECE OF STONE FELL CATCHING MINER'S HAND BENEATH CUTTING RIGHT HAND AND MIDDLE AND RING FINGER.
MINER WAS DRILLING AND SPLITTING STONE. STONE WAS NOT SECURED AND FELL ON HIS RIGHT FOOT, PINCHING IT.
Snowy day employee slipped on rock and twisted his knee. There was no fall or hard impact.
SLIPPED ON ICE IN PLANT WHILE REACHING TO SHUT OFF WATER VALVE INJURING LEFT KNEE.
EMPLOYEE EXPERIENCED A POPPING SENSATION IN GROIN WHILE PULLING WEEDS
Miner ended shift with discomfort in left eye.
MINER FELT FOREIGN DEBRIS IN HIS LEFT EYE
While lifting a load of stone with overhead crane. Four pieces of stone held up by chains. Left arm reached in under load to remove wooden block from load. This caused release of stones onto arm. This caused bruising to the left arm.
