The employee had raised the dump body of Volvo A40G haul truck to dump material into primary dump hopper. In the process of dumping the load the employee claimed the dump body came down quickly from an elevation of six feet while still under load causing the equipment to shake resulting in employee claimed injury of back, buttocks and upper thigh areas.

Slippery Condition due to heavy rain. Employee was rigging a set of steps to lift and while tightening straps the steps jerked and made the employee jump for safety, which made EE slip off a concrete pad and onto pavement. Blunt force from landing on feet made a compound fracture in Left foot.
EE slipped and caught self with right arm tearing bicep muscle.
Tri-axle driver descended cab at employee parking lot area, lost balance, and fell to the ground causing injuries.
EE hand was caught in the line of fire between cooler for crusher #2 and stationary I beam. EE sustained a laceration to left hand required 3 sutures to close the wound. EE had gloves on at the time of incident and work hazard assessment completed highlighting exposures and hazard mitigation plan.
Employee assisted by another employee repositioning roller back into CAT D6 Dozer. Load shifted causing 1 employee to bare burden of load. Employee treated conservatively by athletic trainer and worked at full capacity. Employee continued to have discomfort, MRI issued to discover rotator cuff tear. Surgery now scheduled.
Employee cut (non-gloved) thumb on left hand with razor knife while trying to widen pre-drilled bolt hole of belt skirting resulting in 4 stitches to close open laceration.
AWF welder, repairing rock screen leg for Luck Stone was hit by excavator operator playing on cell phone. X-rays were done that day. Doctor diagnosed bad sprain with bruising and cuts. Employee returned to work on Monday 09/19/2016 but still suffered from a limp.
Employee was pulling a 2" Thick x 10" W x 20' between mid rail and toe board. The board tilted causing hand to become caught between board and mid-rail.
Employee felt pop in right knee as he stepped down onto catwalk adjacent C-2 conveyor.
Employee was welding, and it was hot outside. He wiped his face with a paper towel. Around 8 AM his right eye started to feel irritated. He asked a co-worker to flush his eye. He said it still felt like something was still in his eye. The employee was taken for medical treatment where a small particle was taken out of his eye.
Employee was getting a pipe from a material rack. When getting the pipe the one next to it shifted and pinched his pinky finger between the two pipes.
Employee was working on installing new conveyor belt and fell breaking his foot
Employee exited catwalk to the ground and in that motion sprained his ankle.
Employee attempted to unclog bin with shovel. As he poked up into the bin his hand got caught on the flow switch rod hanging at the bottom of the bin causing a puncture wound between thumb and pointer finger.
Employee was tightening a bolt on a skirt board rubber and the wrench slipped off causing him to bump his elbow on the guard.
Employee was tightening set screw with an Allen wrench when another employee spun head drum causing a laceration to employees left pointer finger.
Employee was assisting in a welding operation applying pressure with a pry bar, when the pry bar slipped causing him to lose his balance. He in turn fell off of the "I" beam which he was standing on, falling 4' 8" to the ground, dislocating his right knee cap.
Right knee popped and caused pain in outside of knee. Knee popped when standing up.
Attempting to install turbocharger on Co Id. #2339 Volvo water truck, when employee felt a pop/burning sensation in left shoulder region.
Miner was bending a belt lacer off and felt a pop in wrist.
Employee was knocking a frozen pin loose, when he hit it with a hammer a peice of metal came up and cut his left cheek. A small peice went into his cheek.
Employee was striking a steel wedge with sledge hammer to pry out a concave at the primary crusher. While swinging the sledge hammer the employee missed the wedge causing strain to his left shoulder.
The miner was securing a rubber screen blank to a rope when he lost grip of the rope releasing the screen. The miner attempted to catch the falling screen blank and put his body into an ackward position causing a strain to neck.
Miner chipped welding slag from bucket and piece of slag got behind safety glasses into Miner's left eye which caused irritation to eye duct.
Employee was walking and stepped on a large rock and sprained his ankle.
The employee stepped into an area that was uneven and full of mud, twisting his right ankle.
The employee was patching a conveyor belt and replacing some bad lacers. The employee reached around and to pull out one of the bad lacers, in which had a sharp edge, cutting his right index finger.
A tresspasser (Non-Company Employee)came on to the edge of the quarry property and committed suicide. The tresspasser was on the edge of the property adjacent to a public road (Quarry Road).
Walking on catwalk. Stepped on rock and twisted right ankle.
Employee was hammering screen on to Deister when hammerhead came off bouncing off screen hitting another employee in the mouth.
The miner was hammering a pin to the hammer on the primary crusher when he struck his finger with the hammer.
The miner was carrying a ladder with another miner, slipped in a mud puddle. The miner caught himself with his left hand, straining his shoulder.
Walked up staircase and ducked under catwalk to check oil in HP crusher tanks. While servicing the crusher tanks with oil, employee stood up under the catwalk and caught shoulder on four inch conduit. Contact was made on back side of shoulder.
A maintenance operation was going on a 8X20 Screen deck which involved cutting. Plywood was placed over the screen media to protect it from damage. The operation was going 4 hours when the fire watch noticed smoke coming from the screen deck. Four fire exstinguishers were emptied to control the fire at which time it grew out of control and the employees evacuated the area.
The miner was bending over to pick up a needle nosed plier off of a bench and proceeded to throw his back out.
While operating a manlift the employee came into contact with a 2 inch angle iron that was located directly above where he was operating the manlift.
A CAT 980 G Loader bumped into the back of a fully loaded Articulated haul truck. The employee that was running the haul was experiencing some stiffness in his neck. There was no damage to either piece of equipment.
The employee was cutting a piece a conveyor belting with a utllity knife and caused a laceration to his right arm.
The employee was walking by a service truck that had a piece of 2 inch angle sticking out of the back of the truck approx. 18 inches and walked into it causing a bruise to his left rib cage.
The individual jumped down from a flat bed truck in which they were unloading equipment from and broke his foot.
Employee went to the MCC room and locked out a conveyor in order to remove rocks from the guard. When removing the material a gust of wind blew dust into employee's eyes. He flushed his eyes out twice after the incident and woke up the next day with his eyes irritated. He reported the incident to his supervisor and was treated and released to work.
Employee was operating the dryer plant when QC requested a sample. Employee went to get a sample from 4" dia. pipe designated in the sampling plant. He was unable to obtain a sample. He removed a 1" diameter bolt, put his right index finger up into a moving screw; his finger was taken off to the first joint by the screw.
Employee was helping to sort screens when the pile of screens shifted and hit his knee as he was trying to get out of the way.
CUTTING FIBERS FROM BETWEEN MULTICONDUCTOR CABLE WITH DIAGONALS. RIGHT HAND CAME UP AGAINST SHARP EDGE OF ELECTRICAL TROUGH CAUSING A PUNCTURE WOUND NEAR THE KNUCKLE OF THE RIGHT RING FINGER, REQUIRING 3 SUTURES. EE WAS NOT WEARING GLOVES AS REQUIRED BY COMPANY POLICY
EE WAS REMOVING A BELL PAN FROM UNDER A 980G. HE TOOK 3 OF THE 4 BOLTS OUT, WHILE REMOVING THE LAST BOLT, PAN FELL DOWN & HIT HIS KNEE.
EE WAS HELPING TO MOVE A BURNER. HE PUSHED THE BURNER TO RELOCATE HIS FINGER WAS SMASHED BETWEEN A WALL AND THE BURNER. (4 STITCHES).
EE JUMPED OUT OF THE UNILOADER AND WAS WALKING WHEN HIS LEFT FOOT TWISTED ON A ROCK AND HE WENT DOWN WE SENT HIM TO THE HOSPITAL.
EE WAS REPAIRING AN EQUALIZER ON DRY BULK TRAILER, EE WAS REMOVING A BLOCK OF WOOD FROM A JACK STAND THAT WAS SUPPORTING THE TRAILER FRAME, THE HYDRAULIC JACK SLIPPED ALLOWING THE FRAME TO DRO P, HIS FINGERS WERE PINCHED BETWEEN THE BLOCK AND FRAME.
