Employee attempted to unclog bin with shovel. As he poked up into the bin his hand got caught on the flow switch rod hanging at the bottom of the bin causing a puncture wound between thumb and pointer finger.

EMPLOYEE WAS CLEANING WINDSHIELD ON LOADER WHEN HIS FOOT SLIPPED ON MUD/GREASE. HE GRABBED THE CAB FRAME TO STOP HIS FALL AND FELT MINOR PAIN IN LOWER BACK. PAIN WORSENED DURING THE DAY AND HE WAS SENT TO THE HOSPITAL FOR EXAMINATION. LUMBAR STRAIN/SPRAIN. HE WILL BE ON MODIFIED DUTY UNTIL NEXT EVALUATION, 20 SEPT. 04. EMPLOYEE ALSO TOLD TO CLEAN VEHICLE STEP SURFACES.
EMPLOYEE WAS LIFTING A RADIATOR WHEN HE FELT PAIN IN LOWER ABDOMEN. HE OBSERVED THAT A PRE-EXISTING HERNIA HAD WORSENED. HE WAS UNSURE IF CONDITION WAS WORK-RELATED. WHEN HE TOLD H.R. MANAGER, ARRANGEMENTS WERE IMMEDIATELY MADE TO HAVE EMPLOYEE MEDICALLY EVALUATED. EMPLOYEE CLAIMS HERNIA EXISTED PRIOR TO EMPLOYMENT BUT NEVER ADVISED THE COMPANY.
While un-rolling spool of 1""cable, the large end retracted back onto the spool. When the EE tried to unwind the cable again, the end sprung free & hit him in the face, knocking several teeth loose. His safety PPE protected his eyes & head from the laceration. It was discovered that 1/2 of the cable spool had been broken either during manufacturing or during shipping.
