Load shifted and pinched EE's 2 fingers on right hand between the love joy plates.

EE hand was caught in the line of fire between cooler for crusher #2 and stationary I beam. EE sustained a laceration to left hand required 3 sutures to close the wound. EE had gloves on at the time of incident and work hazard assessment completed highlighting exposures and hazard mitigation plan.

Struck by powered moving object

AWF welder, repairing rock screen leg for Luck Stone was hit by excavator operator playing on cell phone. X-rays were done that day. Doctor diagnosed bad sprain with bruising and cuts. Employee returned to work on Monday 09/19/2016 but still suffered from a limp.

Employee was striking a steel wedge with sledge hammer to pry out a concave at the primary crusher. While swinging the sledge hammer the employee missed the wedge causing strain to his left shoulder.

The miner was securing a rubber screen blank to a rope when he lost grip of the rope releasing the screen. The miner attempted to catch the falling screen blank and put his body into an ackward position causing a strain to neck.

SUBJECT HAD FINISHED CLEARING JAMS IN SHAKER SCREEN. WHEN MACHINE WAS RESTARTED, A LOOSE ROCK FELL FROM THE BOX OVERHEAD AND STRUCK INJURED ON THE HEAD. HE WAS WAS WEARING SAFETY HARD HAT BUT SUSTAINED CONCUSSION AND POSSIBLE NECK SPRAIN/STRAIN.

EMPLOYEE WAS TIGHTENING BOLT OF SHAKER SCREEN WHEN HE SLIPPED AND FELL, STRIKING RIGHT KNEE AGAINST BOLT AND THE CATWALK. EE FAILED TO WEAR FALL PROTECTION AND FAILED TO ENSURE HE HAD SECURE FOOTING BEFORE BEGINNING TASK. SUFFERED SPRAINED RIGHT KNEE. NO LOST TIME, ON MODIFIED DUTY.

AFTER REBUILDING ENGINE, EMPLOYEE REMOVED SAFETY CABLE ON DUMP BED, STARTED ENGINE & MOVED IT FORWARD. INSUFFICIENT OIL IN RESEVOIR CAUSED DUMP TO SLAM DOWN BEFORE HYDRAULIC PUMP COULD BRING ENOUGH PRESSURE TO HOLD IT UP. EE REPORTED INCIDENT IMMEDIATELY, BUT DID NOT SEEK MEDICAL ATTN. UNTIL 6/13/05. EE SUFFERED A FRACTURE TO THE 12TH VERTEBRA, HE IS ON MODIFIED DUTY.

DURING PRE-TRIP INSPECTION, EE STARTED VEHICLE AND RELEASED THE BRAKE. THE PEDAL SPRUNG BACK FORCEFULLY, STRIKING EE ON THE BOTTOM OF THE FOOT, CAUSING A SPRAINED ANKLE. THE LINKAGE TO THE BRAKE PEDAL WAS BOUND UP AND CAUSED IT TO SNAP BACK UPON RELEASE.

EMPLOYEE WAS WELDING AROUND AN OBSTRUCTION & FAILED TO NOTICE HIS PROGRESS WAS ENCROACHING ON EXPOSED FACE. HE RECEIVED WELDERS FLASH TO HIS EYES. HE RECEIVED EMERGENCY TREATMENT AT HOSPITAL & RETURNED TO WORK. HE WILL BE DISCIPLINED FOR NOT WEARING PPE.

EMPLOYEE WAS DISMOUNTING A SKID STEER LOADER OUTSIDE THE MAINTENANCE SHOP. SHE SLIPPED ON THE EDGE OF THE WET BUCKET SPRAINING HER RT. ANKLE. SHE INITIALLY REFUSED TREATMENT BUT AGREED TO A MEDICAL EVALUATION ON 4/15/04. X-RAYS REVEALED NO BONE DAMAGE. IT WAS RAINING AT THE TIME OF THE INCIDENT.

SECTION OF HAND RAIL ON CR PLANT HAD A STEEL BURR ON THE INSIDE EDGE. SUBJECT WAS USING THE HANDRAIL ON THIS LADDER WHILE DESCENDING FROM THE TAIL OF CONVEYOR 4. HIS HAND SLID ACROSS THE BURR, LACERATING RIGHT PALM. hE WAS NOT WEARING GLOVES AT THE TIME. SIX SUTURES APPLIED AT LOCAL HOSPITAL ER. NO LOST TIME OR MODIFIED DUTY.
