Showing 50 of 93The employee was kneeling down to measure for a new A/C unit when something bumped or pushed the guard into a terminal of the line reactor and caused to arc and burn the employee on the arm and side.
A piece of steel with a lifting eye welded on top of it was sitting in the upright position for lifting. The employee attempted to lay the piece of steel over to keep it from possibly falling over and mashing someone's foot. When EE did this the glove became caught on the lifting lug and EE was unable to get hand away.
Picked up a 50-pound bag of anfo, turned, felt a sharp pain in back.
The employee was operating a skid steer for snow removal. The bucket caught the scale platform and threw employee forward striking the safety bar with employee's chest.
Three miners were cleaning up. One was operating a wheel loader and the other two were throwing scrap steel into the bucket to be taken to a dumpster. The loader operator thought one of the other miners signaled operator to lower the bucket. Operator then lowered the bucket onto one of the miners foot.
Employee was cutting out a wear plate with a torch when hot metal dropped onto boot. The employee did not notice until it burned through boot
At quitting time the employee was exiting EE's service vehicle and twisted EE's left ankle and fell to the ground.

Miner was taking a light off light pole, unhooked the brace the light swung around and hit miner in right ear and head.
